Applicants are invited for full or part time work as a Family Physician to take over a FHO roster of 985 patients. FHT works collaboratively with other members of the health care team to provide continuous comprehensive care to individual patients and their family members. In conjunction with a team of Allied Health care providers, the physician coordinates the patient’s care and helps them navigate through the health care system. The physician develops a comprehensive approach to the management of disease and illness in patients and their families and oversees the care of patients with both simple and complex multi-system problems.

**The physician working as part of the FHT provides the following core services**:
**Health Assessment**
- When necessary, take a full history, including presenting complaint, if any, past illnesses, social history, family history, review of systems, and perform a complete physical exa
- Periodically taking a specific history and performing a physical exam as required to screen patients for disease
- Regularly taking a specific history and performing a physical exam as required to respond to patient complaints and/or to manage chronic problems

**Diagnosis and Treatment/ Management**
- Provision of continuous care to patients based on the delivery of the following services
- Acute medical treatment for a range of medical problems from minor ambulatory care visits to severe life threatening illness
- Primary reproductive care
- Primary mental health care
- Service coordination and referral
- Patient education and preventative care

**Job Description of Family Physicians**

Provide primary care to individuals and their families taking into account risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable, their families

Perform clinical duties by seeing and assessing client for routine health and illness care, prescribing treatment, during scheduled hours of service.

Work with the health care systems, to support the health needs of clients and community groups obtained through client encounters and other community contacts

Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, and answering client inquiries and directing them to or providing appropriate treatment.

Maintain and develop professional competence through appropriate continuing education (i.e., peer interaction, literature review, conferences, courses, continuous learning)

**Qualifications**:
Medical degree and family medicine residency from a recognized university **CCFP Certification from the College of Family Physicians of Canada (CFPC) **License to practice from the College of Physicians and Surgeons of Ontario (CPSO) **Fluency in English, both oral and written**

- Monday to Friday

Work Location: In person

**Salary**: $216,000.00-$300,000.00 per year
