**ABOUT THE ROLE**

Reporting to the Youth Wellness Hub York Region Manager, the Youth Wellness Intake Worker will be part of a diverse multidisciplinary Youth Wellness Team. Serving as the initial point of contact for the Youth Wellness Hub of York Region, creating a welcoming environment for a diverse population. This role will support a consistent and centralized intake process for all services, including championing the use of YWHO’s electronic client information system and screening tools. The Youth Wellness Intake Worker will provide information pertaining to diverse services within York Region and support in the coordination of day-to-day operations, which will include liaising with community partners for scheduling and programming events.

**Employment Status**:Permanent Full-Time**

**Location**: This role will initially be based at 402 Bloomington Road (Aurora), with a planned transition to 9401 Jane Street (Maple) in summer 2025. Flexibility between York Hill sites will be required during the transition period, including travel within York Region.

**Hours of Work**: Thirty-five (35) hours per week, including evenings and some weekend work may be required.

**Salary Range**: $58,294 to $61,842

**A LOOK AT YOUR DUTIES AND RESPONSIBILITIES**:

- Oversee the intake process for youth and families, including the use of YWHO’s electronic client information system and appropriate screening tools (My Wellness Passport).
- Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe, and welcoming hub atmosphere.
- Serve as a representative of YWHO to youth, families, and service providers, providing support and orientation to the Hub. This includes introducing YWHO’s electronic client information system and measurement-based care approach to ensure seamless access to services.
- Support the transition of youth and families between various Youth Wellness Hub service pathways (Skills and Well-being, Community & Social Support and Clinical Services).
- Collaborate with the Youth Wellness Team and other hub service providers to provide seamless access to support youth.
- Provide support in facilitating skills and well-being activities both individually and within group formats as needed.
- Support engagement with youth and their families to support ongoing access to services, including gathering feedback on all hub programs and services.
- Provide services and support to youth and their families in a way that is consistent with the values of the YWHO Model.
- Participate in monthly YWHO Provincial Community of Practices.
- Provide access to services in-person and remotely as needed.
- Perform other duties as assigned.

**WHAT WE ARE LOOKING FOR**:

- Completion of a post-secondary education in social services and/or a combination of education and experience.
- Minimum three (3) years of experience working with children and youth in a community-based mental health and/or substance use setting.
- Excellent organizational, planning and time management skills and the ability to work in a dynamic and fast-paced multidisciplinary environment.
- Excellent interpersonal, verbal and written communication skills, including editing skills with attention to detail.
- Knowledge of York Region community resources available to support youth and families across a continuum of care and holistic understanding of health and mental health.
- Experience in supporting safe and meaningful participation and being open to diverse ways of thinking about, understanding and articulating challenges related to mental health and substance use is required.
- Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQ+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
- Experience with health information systems is an asset.
- Must be fluent speaking, writing, and reading in English.
- Ability to work under pressure in crisis situations.
- A willingness to work flexible hours which include evenings and some weekend work; comfortable working in office and in community-based settings.
- Awareness and appreciation for the dynamic of equity issues (diversity, equity and inclusion), especially as it relates to gender, ethnicity, race, and economic status.
