**Salary**:$25.00-$28.00/hour plus benefits

**Location**:Mostly remote. Travelling around the lower mainland with occasional meetings at our North Vancouver Head Office

**Position Summary**
The Outreach Support Worker will work collaboratively within a dedicated team to provide targeted support to families. This includes managing a caseload, creating and implementing flexible case plans, and maintaining professional and supportive relationships with clients. The role requires a strong focus on client-directed goals, crisis management, and collaborative efforts with community resources.

**Key Responsibilities**:

- ** Relationship Building**: Establish and maintain professional, supportive relationships with families aligned with program goals.
- ** Case Planning**: Design individual, proactive case plans that include measurable goals and encourage client participation.
- ** Client Support**: Provide direct support for issues related to parent/child or teen conflict, and crisis management.
- ** Caseload Management**: Dedicate 50-70% of full-time hours to direct service, with minimum weekly client contact.
- ** Documentation**: Complete administrative duties including monthly reports, progress assessments, critical incident reports, and discharge reports.
- ** Collaboration**: Engage in integrated case planning and maintain relationships with community resources and collateral agents.

**Reporting & Supervision**:

- Directly report to the Senior Program Manager, Outreach.
- Participate in program and team meetings.
- Attend regular supervision meetings.
- Submit reports and incident updates promptly, in alignment with program guidelines.

**Qualifications**:

- ** Education/Experience**:

- Bachelor's degree in Child & Youth Care (BCYW) or Social Work (BSW) with 3 years of family or foster support experience, or
- College diploma with 5 years of relevant experience, or
- A combination of education and 7 years of family/foster support experience.
- ** Additional Requirements**:

- Access to a vehicle with a valid BC Driver’s License and business insurance with $3,000,000 liability.
- Acceptable Criminal Record Search report.
- Current First Aid and CPR certification (we will train those who require this)
- Current CPI or MANDT certification (we will train those who require this)
- Strong verbal and written communication skills.
- Experience in parent education, strategy development, mediation, counseling, advocacy, community networking, and life skills management.
- Ability to collaborate with community resources and within a team.
