**BSO Inpatient Behaviour Intervention Specialist (“BIS”)**

The Behaviour Intervention Specialist (“BIS”) will support the staff and informal caregivers, providing care to older adults located in the hospital sector who have complex and challenging responsive behaviours that are unmanaged and are deemed as a barrier to being able to provide care and a potential barrier to discharge. These behaviours may be associated with Alzheimer’s disease and related dementias, mental health, substance use, and other neurological conditions.

**Key Accountabilities**:
- Collaborate with PSWs, nurses, Drs., OT/PT, all relevant staff to conduct behavioural assessments as set out in Alzheimer Society Peel best practice guidelines.
- Implement, evaluate and document specific behavioural interventions in consultation with Acute Care Behaviour Consultant.
- Review all relevant documentation as it pertains to behaviours and care: DOS, progress notes, incident reports, etc.
- Direct observation of patients during times of reported responsive behaviours to identify triggers/unmet needs, to recommend appropriate strategies to manage the behaviours; this may include ostomy/colostomy care with assigned nurse, medication administration, feeding assistance due to low oral intake, lap-tray/and/or seat-belt trials, personal care etc.
- Develops, monitors and evaluates the BSO care plan recommendations by; reviewing documentation, behavioural tracking and outcome measures to illustrate impact.
- Provides 1:1 transition support during hospital discharges: Accompanying Patients in non-Emergency vehicles to ensure behavioural stability during transport, huddling with staff in the receiving facility
- Provides patient specific education, training, modeling and coaching to hospital staff involved in patient care to facilitate understanding and management of responsive behaviours.
- Creates appropriate referrals to additional BSO team members where needed to support the patient, caregiver and staff.
- Escalates complex behavioural transition cases to Acute Care Behavioural Consultant to mitigate the risk of delayed or unsuccessful transition from acute care.
- Provides support to ensure the successful transition of patients from the hospital to their next transition location
- Participates in care conferences/patient huddles and Joint Discharge planning meetings as required
- Maintains ongoing contact with SDM, POA and medical health care professionals.
- Initiates and participates in ongoing collaboration with community partners
- Maintains accurate and timely monthly statistics through the identified Statistical Reporting System
**Qualifications and Skills**:
- College/University Degree in a relevant health or social science field; training in Gerontology or equivalent
- Minimum 3 years of work-related experience; Experience working in an acute care setting is an asset*
- Minimum 3-5 years of experience working with the elderly with Alzheimer's Disease and other related dementias
- Working knowledge of various regulatory requirements within the Health Services environment
- Behavioural intervention experience
- U-First, P.I.E.C.E.S, GPA Certification
- CPR & First Aid Certificates
- Intermediate to Advanced - Word, Excel, PowerPoint & Outlook
- Strong written and oral communication skills
- Self-directed, team player
- Valid driver's license and access to a reliable car
