Lpn - Client Care Coordinator for Homecare

This job is for an LPN who does not prefer the daily grind in a facility type setting and who has an empathetic personality and prides in helping primarily seniors age in place with homecare assistance in the comforts of their own home setting.

Conduct intake calls from prospective clients, arranges In-Home Assessment Visits to promote acceptance of service, completion of necessary documentation and develop a Plan of Care for new clients.

Ensures the quality and continuity of care of clients by routinely interacting with clients, family members, community providers/ referral sources, case managers to optimize the effective care of the client.

Contribute to the close knit office family through input on caregiver selection, recommendation, training in the field as well as supervisory visits with caregivers who we call Comfort Keepers.

**Qualifications**
- Valid LPN license
- Strong patient care and nursing skills
- Proficient in medication administration
- Knowledge of medical terminology
- Experience in home care, acute care, and dementia care
- Background hospice care, or senior care is a plus

Pay: $26.00-$28.00 per hour

Expected hours: 40 per week

**Education**:

- AEC / DEP or Skilled Trade Certificate (preferred)

Licence/Certification:

- LPN/RPN (required)
- Nursing (required)
- CPR Certification (required)
- First Aid Certification (preferred)
