**Title**: Community Services Planner
**Status**: Permanent Full-Time
**Hours of Work**: Monday to Friday; 35 hours per week, some evening and weekend availability required
**Starting Rate of Pay**: Starting pay rate $27.70 per hour.

As part of our ‘Let’s Go Home Program’, Community Services Planners serve as the first point of contact for individuals requiring intensive supports in the community upon discharge from the hospitals in London-Middlesex. Working closely with the individual, hospitals, Home and Community Care Support Services as well as other community partners, the Community Services Planner provides service navigation and referrals for the client and their caregivers and assists them in understanding and accessing available services and supports. The Community Services Planner completes the intake process, provides care planning and schedules service delivery for Community Support Services while working collaboratively with the system partners.
**Role Requirements**
- Possess a relevant post-secondary school diploma/degree and a minimum three years’ experience with an in-home/community care organization or social/health service program
- Knowledge of the London Middlesex Community Support Services providers is an asset
- Ability to provide high-quality customer service in a fast-paced environment with multiple priorities and competing deadlines
- Demonstrated problem solving, assessing situations, setting priorities and critical thinking skills
- Working knowledge of the InterRAI preliminary Screener and InterRAI CHA are an asset
- Excellent interpersonal and communication skills; both written and oral
- Demonstrated proficiency using the Microsoft Office suite of programs, including Word, Excel, and PowerPoint
- Valid Driver’s License, insurance and use of a vehicle
- Must possess (or able to acquire) a current Police Vulnerable Sector Check
**Role Responsibilities**
- Working as the first point of contact, the Community Services Planner will work closely with the hospital and Home and Community Care Support Services in order to determine individual’s eligibility for service and to inform service set up
- Understands and adheres to the privacy policy of the organization when working with client information
- Participates in discharge planning, assessment of clients needs and completes home risk assessment
- Communicates with funders, clients and care teams and coordinates with program staff to process referrals, initiate services, implement service delivery changes, and address/ respond to phone inquiries from community, customers, clients, doctors, service providers and other agencies
- Receive and Coordinate all standardized documentation, data collection, triaging of calls.
- Schedule service delivery for community support services.
- Completes the Preliminary Screener for the RAI to identify needs for further client assessment and completes assessment as required
- Prepares and sends out client service agreements, consents and program information.
- Works collaboratively and maintains positive relationships with partner agencies
- Accountable for being knowledgeable of all policies and procedures related to their employment and functions.
- Maintains statistical reporting of service as required by the funder
- Performs other tasks as assigned
