**This is More Than a Job—It’s a Mission**
Imagine a role where every conversation you have can change a life. Imagine standing at the forefront of health equity, ensuring that families, elders, and newcomers receive the information and support they need to make the best decisions for their well-being. Imagine being the trusted voice your community turns to in times of uncertainty.
This is what it means to be a **Community Health Worker (CHW).**
This role is for those who refuse to stand by when their community faces barriers to healthcare. It is for those who believe that **knowledge empowers, awareness transforms, and trust heals.** It is for those who want to be **part of a movement that strengthens communities, saves lives, and builds a healthier future for all.**
If you have ever wanted to **be the change**, this is your moment.
**About CentreCanada and the Immunization Partnership Fund**
CentreCanada is a non-profit organization committed to **uplifting immigrants, refugees, and newcomers** by providing culturally responsive health, education, and integration services. Through this initiative, we are ensuring that vaccine education and health literacy reach the people who need it most.
With support from the **Immunization Partnership Fund**, this program will help increase vaccine literacy, confidence, and access within diverse communities. **Your role will be critical in making this vision a reality.**
**What You Will Do**
- **Lead outreach and education efforts** in your community, ensuring that families, parents, and elders have access to reliable vaccine information.
- **Organize and facilitate community forums** to address concerns, answer questions, and foster dialogue on immunization.
- **Develop and deliver health literacy workshops** to parents and caregivers, equipping them with the knowledge to make informed health decisions.

This is not a role where you wait for change to happen. **You create it.**
**Who You Are**

**Why This Matters**
Many people struggle to access the healthcare information they need—not because they don’t want to, but because the system was not designed with them in mind. Language barriers, cultural differences, and misinformation create **gaps in trust and understanding.**
You will help **bridge that gap.**
You will be the **link between the community and healthcare providers.** You will help ensure that **no one is left behind** when it comes to making informed health decisions. You will be **a trusted voice in a sea of uncertainty.**
