Interior Health has an exciting permanent full time opportunity for a Masters Level Social Worker to join the West Kelowna Urgent Primary Care team in West Kelowna, B.C.
**What Will You Work On?**
The Primary Care Social Worker functions as a member of the interdisciplinary team and applies best practice to provide comprehensive assessment, care planning and intervention for people and their families. Working collaboratively with other health care providers and the interdisciplinary team, the Social Worker provides direct psychosocial care, counselling and group work, system navigation and advocacy, education and linkage to relevant specialized or community resources while facilitating client self-management. The Social Worker is responsible for providing social work services to maintain or improve the health and functioning of persons, families and communities.
Some other responsibilities include:
1. Provides direct social work services and care coordination to people, families, groups and communities by performing duties such as carrying out formal and informal assessment of people’s needs and preferences in collaboration with physicians, and members of the interdisciplinary team, in accordance with applicable guidelines and policies and utilizing established assessment tools.
2. Works collaboratively with clients, community support and interdisciplinary team in meetings or conferences in developing and implementing appropriate individualized treatment plan. Discusses clients’ assessments, providing input from a social work perspective on a variety of care issues related to the client to ensure proper and effective treatment of the client.
3. Provides solution focused short term counselling for individuals, families and/or group counselling mediation and conflict resolution services and crisis interventions by methods such as exploring emotional responses to illness/injury in order to identify problems, personal concerns and treatment objectives and to facilitate change in coping style, behavior, attitude, feelings and adaptive responses to an adverse life event.
4. Assessing the person’s response to interventions, reassessing the effectiveness of interventions and modifying plans of care when appropriate.
5. Coordinating agency investigations and responding to community and internal referrals around abuse/neglect of vulnerable adults: providing assessment, direct counselling; developing support and assistance plans for vulnerable adults and their support persons and maintaining confidential records of reports and investigations of abuse/neglect; determining if an adult is capable of giving consent to health care: obtains substitute consent form a temporary substitute decision maker (TSDM), in accordance with the Health Care (Consent) and Care Facilities (Admissions) Act.
6. Participates in Quality Improvement Initiatives.
7. Contributes to the development and revision of standards of care, policies, and procedures.
8. Performs other related duties as assigned.
Shift times are from 08:00 to 16:00 hours.

Education, Training and Experience:
Master’s Degree in Social Work from an accredited university.
Two years recent related experience in a healthcare, or a combination of related education, training and experience.
Current registration with the British Columbia College of Social Workers.
Current valid BC driver’s license.
