**Bilingual Social Worker**

**This is to staff one permanent, full-time position.**

**Job Summary and Requirements**

This position functions in a collaborative practice model as part of an interdisciplinary team providing comprehensive primary health care services to Centre de Santé Constellation Health Centre (CSCHC)patients.

Reporting to the Clinic Manager, this position develops and provides social work services to support the wellbeing of patients at the clinic. This includes case management and coordination supports, health system navigation, liaising with and referring to other health and social service providers, educating and providing supportive counselling.

**Note: English is the administrative language of work in the Government of Yukon and therefore your resume must be submitted in English.**

**Essential Qualifications**:

- Bachelor of Social Work.
- Demonstrated experience providing social work services in an interdisciplinary team, preferably in a primary health care environment.
- Registration in good standing with a Canadian College of Social Work would be an asset

**Desired Knowledge, Skills and Suitability**:

- Knowledge and experience providing care planning, supportive counselling, crisis intervention, and case coordination.
- Knowledge and experience providing culturally safe and trauma-informed care.
- Strong knowledge and experience in social work principles and practices.
- Demonstrated capacity to complete professional assessments.
- Excellent ethical decision-making skills and practice guided by the Canadian Association of Social Work Code of Ethics.
- Ability to work independently and as part of an interdisciplinary team.

**Additional Information**:
**This is an imperative designated bilingual position. A condition of employment is that you have and maintain a sound knowledge and command of English and French, and that you are able and willing to function satisfactorily in both languages**

**Conditions of Employment**:

- TB Screening;
- RCMP Vulnerable Sector Security Clearance;
- Valid Class 5 Driver's Licence or equivalent;
- Verification of Bachelor of Social Work degree; and
- Have and maintain a sound knowledge and command of English and French, that you are able and willing to function satisfactorily in both languages and successful pass the language proficiency level required for the position.

**Job Requirements**:

- Willingness to travel both within and outside of the Territory.
- May be required to participate in an after hours schedule to support clinic services outside of regular business hours.

Relevant du gestionnaire de clinique, la personne titulaire du poste développe et offre des services de travail social pour assurer le bien-être de la clientèle du Centre. Cela comprend le soutien à la gestion des cas et à la coordination, la navigation dans le système de santé, la liaison avec d’autres fournisseurs de services de santé et de services sociaux et le renvoi à ces derniers, l’éducation et la prestation de services de counseling.

Pour réussir dans ce poste, la personne doit idéalement avoir de l’expérience dans le soutien des professionnels de la santé et de la patientèle d’une clinique tout en contribuant à un environnement inclusif, non discriminatoire et sûr sur le plan physique, psychologique et culturel.
