Requisition ID: 128961 Opportunity Type: Locum to Permanent Estimated Annual Salary: 100,000 - 150,000 Type of Remuneration: APP- Alternative Payment Plan Site Visit & Reallocation Allowance Programs: **About This Opportunity**: **Responsibilities**: Responsibilities include but are not limited to: - assisting in development and delivery of inter-professional initiatives in Palliative Care across all faculties - provided care in all care settings in the area including hospital, in-patient palliative care unit, home, LTC, and assisted living residences - follow patients after initial consult with appropriate visits, phone support and collaboration with the patient’s other health care providers, in a manner that is most supportive of each family physician and patient - work closely with the palliative care team - act as a patient’s advocate - prescribe and administer medications and treatments, CADD pumps usage - mentor longitudinal integrated clerkship students and family medicine residents from Dalhousie University **Expected Hours**: - part-time 0.5 FTE **Qualifications**: - Eligible for Licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS) - Eligible for certification with the College of Family Physicians of Canada (CFPC) - Canadian Medical Protective Association (CMPA) - accredited training in Palliative Medicine or Certificate of Added Competence in Palliative Care - demonstrated ability and experience working in a shared care model and across a multitude of care settings - demonstrated interest in the palliative approach to care and capacity building with other colleagues **Community Details**: **The South Shore of Nova Scotia provides wonderful way of life.
