**Job Description**: **POSITION PURPOSE** Reporting to the Manager, Child and Family Health, and as a member of the Healthy Babies, Healthy Children (HBHC) home visiting team, provides activities that support families who are pregnant and/or with young children at high risk of not meeting developmental milestones, within the guidelines of the Ontario HBHC Program, including home and community visiting, telephone and family support activities. **MAJOR RESPONSIBILITIES** - Works as a part of a team to identify service plan goals and strategies, implement activities and monitor and review family progress. - Develops trusting, non-judgemental and supportive relationships with children and families; encourages and assists families to identify and build on existing strengths. - Role models positive parenting skills, effective communication, problem solving and coping skills that promote family well-being. - Links individuals and families to appropriate community supports and services (e.g. food banks, Ontario Early Years Centres, parenting groups and recreation programs). - Consults with and/or reports to the Children’s Aid Society, in accordance with responsibilities under Ontario’s Child and Family Services Act. - Advocates for families to receive needed services/supports. - Provides care that is respectful to the diversity within York Region. - Identifies safety concerns and provides information and support that address safety needs/ requirements of children and families. - May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan. - Performs other duties, as required, in accordance with job responsibilities and program objectives. **QUALIFICATIONS** - Successful completion of a Post Secondary Certificate or Diploma (1 year or more) in health, social services, child development, early childhood education or related discipline, or approved equivalent combination of experience and education. - Minimum three (3) years experience in early childhood settings providing guidance to families about parenting or experience working with infants or young children. - Satisfactory Police Criminal Background Check. - Valid Ontario Class "G" driver's license and free of serious offences under the Highway Traffic Act. - Demonstrated ability in the Region’s core competencies.
