**Description of Duties and Responsibilities**

The Social Worker reports directly to the Program Manger and is supervised in daily case planning. The incumbent provides specialized trauma-informed daily functioning support services, as well as general case management services to clients as a member of a multi-disciplinary outreach team (i.e. Spady CAT).

This front-line position provides direct support and clinical services, makes referrals to clinical specialists and advocates for appropriate clinical services and policies. The position also manages a designated caseload on behalf of the Team and respectfully participates in joint Team service planning and case reviews. Social Worker supports the Team, develops, and maintains a network of community resource and service providers, is committed to on-going education and training, respects client and staff confidentiality and adheres to rigorous ethical standards. This is a position in a mobile outreach team and many services are provided in client’s homes and communities.

**Specialized Client Services**
- Orient and motivate clients to participating in appropriate daily functioning, life skills, physical health mental health and substance use support services;
- Make referrals to and support client engagement with relevant clinical specialists;
- Develop and maintain a network of community service providers in the field of daily functioning;
- Provide supported referrals to external service providers;
- Assess health, mental health, substance use and functional challenges within scope of practice;
- Research and participate in relevant training opportunities.

**Case Management**
- Orient and motivate clients to participate in appropriate Spady CAT services;
- Track and document client progress through Episode of Care, internal databases, and case files;
- Liaise with and provide on-going support to referring agencies and client support workers regarding clinical issues;
- Arrange and provide a significant portion of services in client’s homes and communities.

**Multi-Disciplinary Team Involvement**
- Contribute to joint client service planning and case reviews;
- Provide specialized expertise and service supports to other staff’s caseloads;
- Assist in the development and preparation of Team resources and materials;
- Supervise student placements as assigned;
- Other duties or tasks as required by the Team Director or Clinical Supervisor.

**Position Requirements**
- A minimum of a Bachelor’s degree in Social Work with 3 to 5 years of experience providing services to clients;
- Understanding of Truth and Reconciliation Commission, the impact of colonization in Canada, and the familiarity with the epidemic of missing and murdered Indigenous women in Canada;
- Good standing in registration with regulatory bodies, as appropriate;
- Strong understanding of mental health resources in the community and an ability to develop and foster community links and networks aligned with community mental health integration;
- Familiarity with and supportive of harm reduction, Housing First and recovery practices;
- Knowledge of trauma-informed service provision approaches and best practices;
- Able to use their own vehicle and purchase a minimum of $2 million in liability coverage;
- Able to submit a current Police with Vulnerable Sector check;
- Able to travel to meetings and appointments;
- Able to work as a respectful and engaged member of a multi-disciplinary team;
- Demonstrated level of maturity, including ability to work in an independent and ethical manner respecting staff and client confidentiality;
