The System Performance, Quality & Research Lead works as a key member of the Nephrology & Diabetes teams to lead quality initiatives and projects to support the provision of effective, evidence informed quality care. Reporting to the Director of Regional Nephrology & Diabetes Programs, the System Performance, Quality & Research Lead supports Ontario Renal Network (ORN) and corporate initiatives and strategies with the goal of increasing efficiencies and clinical analysis in line with the ORN and organizations strategic plan.
The role works closely with the Program Director, Medical Director, Managers, Clinical Practice Leaders, to lead, guide the key quality/strategic and performance improvement initiatives identified as priorities in the current and future Ontario Renal Plans (ORP) to meet and exceed established performance metrics from the Ontario Renal Network (ORN). The role integrates strategy, quality, practice and project planning across all services and sites within Nephrology/Diabetes and with external partners for initiatives, e.g. Ontario Health (OH), ORN and third party providers to achieve the deliverables of the ORN and excellence in patient care.
The System Performance, Quality & Research Lead coordinates and leads the strategic priorities of the Ontario Renal Plan within the program and provides the Program Director and Medical Director with guidance on standards and structures for monitoring and reporting quality and data metrics, including data analysis to meet and exceed the performance metrics with the ORN and OH for Nephrology and Diabetes care.
The System Performance, Quality & Research Lead will contribute to the implementation of continuous improvement in the program to achieve excellence in person-centered care and meet the standards of the ORN through the utilization of skills and attributes associated with the fields of process improvement, project management and change management in an integrated manner.
DUTIES & RESPONSIBILITIES:
Supporting the Director, Nephrology/Diabetes the System Performance, Quality & Research Lead duties include, but are not limited to:
- Prepare and analyze statistical reports/surveys from the ORN and Ontario Renal Reporting System (ORRS) to identify trends and issues requiring quality improvement focus.
- Monitor progress against the ORN quality and performance metrics in preparation of the quarterly performance reviews with ORN.
- Monitor progress against the Diabetes quality and performance metrics.
- Facilitate the implementation of quality improvement activities and acts as a knowledge expert and resource to leaders in the development and maintenance of quality improvement processes
- Works with program leadership to ensure standardized and consistent approach to process improvement, change management, and project implementation
- Liaise with Patient Experience to identify trends corporately that will align with the person-centred care strategic initiatives and deliverables of the ORN and support action planning for improvement at the program level
- Contributes to the development, implementation and management of initiatives/projects related to corporate annual business plan that can be aligned with the program specific annual projects and improvement goals. Monitor progress against the ORN quality and performance metrics in preparation of the quarterly performance reviews with ORN
- Coordinate the delivery of appropriate education, mentoring and coaching in support of quality improvement activities across the programs (Nephrology and Diabetes) using established principles such as LEAN, Six Sigma, project management, and quality improvement methodologies
QUALIFICATIONS:
- Bachelor's Degree in a related clinical field, Bachelor's degree in a Healthcare Administration, (Master's Degree in a health care related field preferred)
- Membership in good standing within a Regulated Health Discipline
- 3 years recent, progressive management experience in related health care services, including a minimum of two years' experience in quality improvement development, implementation and evaluation
- Proven, recent, significant clinical experience in Chronic Disease Management populations; and knowledge and experience in programs for patients with chronic conditions is required;
- Knowledge and applied experience in Ontario Renal Plans and ORN performance metrics required; Ontario Renal Reporting System (ORRS) preferred;
- Demonstrated working experience with EPIC preferred;
- Educational preparation in quality improvement, certificate in quality improvement or equivalent in the use of quality improvement methods and tools
