Home and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement.
These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.
Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.
The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.
Position Summary
Reporting to the Manager, Home and Community Care; the Direct Care Facilitator coordinates Medical Assistance in Dying (MAiD) requests, processes, facilitates and authorizes referrals for our Direct Care Nursing teams, reviews and manages direct care referral waitlists, and facilitates submission of patient care documents to external partners as required for patient flow. The Direct Care Facilitator also acts as an information resource to internal and external stakeholders.
**Duties will include, but not be limited to, the following**:
- Receives and monitors referrals for Direct Care nursing within the HCCSS South East geography through our CHRIS system
- Ensures that information regarding waitlists is kept current on the data base for Direct Care Nursing programs
- Contacts patients/families/primary care partners as required for initiation of referrals and services for MAiD
- Coordinates referrals for MAiD, TSN, RRN and related programs
- Completes and organizes documents and submissions from Nurse Practitioners that are submitted to external partners
- Documents admissions according to policies and procedures
- Liaises with Direct Care Nurses, Team Assistants, Care Coordinators, and other internal staff to resolve crisis situations
- Attends and participates in Direct Care Nursing and MAiD update meetings, as required
- Prepares and maintains statistics of referrals and admissions to Direct Care Nursing programs in HCCSS South East.
- Closes files on an appropriate and timely basis.
- Performs other duties as assigned
Minimum Qualifications & Position Requirements
- Current registration with the College of Nurses of Ontario as an Registered Practical Nurse (RPN)in the general class
- Health Care experience in medical administration, or equivalent HCCSS South East experience is preferred
- Knowledge of PHIPA, Substitute Decision Act, and Health Care Consent Act
- Strong data entry skills and ability to work with MS Word, Outlook, Excel and the Internet
