**Job Description**:
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
The Rehab Navigator is a member of the interdisciplinary team and is responsible for the coordination (review and triage) of referrals to determine whether patients meet admission criteria to the inpatient rehabilitation programs across Island Health, in consultation with the inpatient rehabilitation unit Clinical Nurse Leader (CNL)/teams, including Physiatry where applicable. The Rehab Navigator possesses an overview of the patient's needs, services and resources and facilitates the early inpatient rehabilitation journey.
The Rehab Navigator works in a team based environment and provides consultation to the referring unit clinical team; establishes and maintains inter-agency/community communications/relationships with external agencies; assesses patients, participates in program evaluation and quality improvement activities/committees for designated program; maintains a variety of records; chairs designated meetings/committees; prepares and delivers presentations.
**QUALIFICATIONS**:
**Education, Training And Experience**:
Graduation from a recognized university program in Occupational Therapy; and
Current licensure in a practicing category with the College of Health and Care Professionals of BC as an Occupational Therapist; and
Eligible for membership in the Canadian Association of Occupational Therapists (CAOT).
Five (5) years' recent, related experience working in a rehabilitation setting including three (3) years' experience with the neuromusculoskeletal, and/or complex orthopaedic/frail elderly populations.
An equivalent combination of training and experience may be considered.
OR
Graduation from a recognized university program in Physiotherapy; and
Current practicing full registration with the College of Physical Therapists of BC; and
Eligible for membership in the Canadian Association of Physiotherapists (CPA).
Five (5) years' recent, related experience working in a rehabilitation setting including three (3) years' experience with the neuromusculoskeletal, and/or complex orthopaedic/frail elderly populations.
An equivalent combination of training and experience may be considered.
**Skills And Abilities**:
- Comprehensive knowledge of evidence-based clinical practice related to rehabilitation and brain/neuro and musculoskeletal injuries/conditions, current trends and practice.
- Comprehensive knowledge of short and long term impact of MSK/Orthopaedic/Arthritis/Brain Injury/Neurological/illness on individuals, their families and their community and learning the needs of those caring for and living with affected individuals.
- Comprehensive knowledge of the standards of practice and guidelines for clinical practitioners.
- Broad knowledge of other health care disciplines and their role in patient care.
- Knowledge of community resources.
- Demonstrated ability to communicate effectively with co-workers, physicians, other health care team members, patients and their families, either one-on-one and in groups.
- Demonstrated ability to function as a team member and facilitator.
- Demonstrated ability to provide effective consultation.
- Basic computer literacy including knowledge of the EHR and Sharepoint platforms.
- Demonstrated ability to provide support and guidance to community partners and patients/families.
- Comprehensive knowledge of the organization's policies, procedures and standards of care.
- Demonstrated ability to establish workload priorities in collaboration with others in a complex health care environment.
