For more information on WPSHC, please visit our website. **About the Opportunity**: We are looking for a **Geriatric Assessor and Care Navigator** to join our inter-disciplinary team of skilled professionals. Reporting to the Clinical Manager, the Geriatric Assessor and Care Navigator facilitates inpatient care and interventions of older adults and assists in the continuation of identified care needs in the community upon transition out of hospital. The Geriatric Assessor and Care Navigator provides thorough Geriatric Assessments and interventions in collaboration with the inpatient care team, which includes Physician Assistants and MRPs, allied health partners, Nurses, and PSWs. The assessments and interventions will be developed into a care plan to reduce disability, provide optimal patient-centered care, and assist in a management plan for co-existing chronic conditions. This position will work day shifts, primarily Monday to Friday. **About the Benefits** In return for your dedication, you will be rewarded with an attractive annual salary of $87,000 to $111,000 based on experience. This is an opportunity **to grow in an established and highly respected organization**, using your experience and skills to inspire a direct and positive impact on the community. **More About You**: **Basic Requirements** - A degree in a registered health care discipline (e.g., RN, OT, PT) with substantive experience and training in geriatric care (e.g., Senior Friendly Care, Geriatric Syndromes, Frailty); - Minimum five (5) years of recent and relevant experience, preferably in a geriatric care setting. - Previous experience as a preceptor, mentor, or clinical instructor; - Knowledge of adult learning principles. - Possess knowledge of Senior Friendly Care and Alternate Level of Care (ALC) Best Practices; - Demonstrated background in critical decision making, a leadership style that facilitates and promotes teamwork and the ability to motivate others; - Knowledge of roles within the interdisciplinary team. - Ability to speak to a small group or on a one-to-one basis using appropriate vocabulary and grammar and the ability to speak and interact with persons of various social, cultural, economic, and educational backgrounds; - Skill in writing concise, logical, grammatically correct correspondences and reports; - Skill in supervising staff, including the ability to delegate responsibility, organize objectives, and evaluate subordinate effectiveness; - Ability to work well under pressure and use good judgment in assessing difficult situations; - Aptitude for proactive problem solving, using strong critical thinking, and negotiating skills; - A demonstrated ability to develop, deliver and evaluate training and coaching efforts; - Demonstrated satisfactory attendance levels, performance levels, disciplinary history, and positive work references. **_Please note: _**_For the safety and well-being o
