***:To participate in the Inter-Disciplinary Geriatric in Patient Consult Team to provide expert consultation and treatment to patients over the age of 65, centered in comprehensive geriatric assessment.

**POSITION RESPONSIBILITIES**
- This team functions in an interprofessional model where all team members act as Geriatric Assessors, sharing a common set of competencies.
- To continually evaluate the client’s treatment program and adjust the treatment program so that it meets the client’s goals.
- Applied skills in acute management of geriatric syndromes, including delirium, polypharmacy, falls and functional decline, acute/sub-acute cognitive changes, bowels/bladder, and nutrition/hydration problems.
- Applied knowledge of chronic disease management in the older population.

**POSITION REQUIREMENTS**:

- Must have a minimum of 4 years Bachelor degree or Masters Degree in health related field from an accredited university (for example; Bachelor of Social Work, Masters of Science and Physiotherapy, Masters of Speech Language Pathology, Masters of Occupational Therapy or other equivalent)
- Current registration in good standing with relevant regulatory College.
- Must possess the verbal ability to communicate effectively with clients, families and other professional personnel.
- Demonstrated person and caregiver-focused care, supported by evidence based clinical practices.
- Must possess the ability to work with mínimal supervision and must follow standardized procedures. Should be adaptable to cope with changing contacts with people, situations and conditions.
- Must have employment history, which demonstrates regular attendance at work.
- Bilingualism (French and English) would be considered an asset.
