**Role Overview**

The role of the Supportive Housing Worker is crucial for supporting tenants in maintaining their housing, developing independent living skills, and pursuing personal recovery and wellness.

**About the Teams**

Bailey House is a supportive housing program that provides rent-subsidized accommodation and support for 17 men living with mental health challenges and co-occurring diabetes or pre-diabetic conditions. Each tenant has his own bedroom, a bathroom, and shares a communal kitchen with other tenants.

33 Beaty is a nine (9) room rooming house located in Parkdale, offering deeply affordable housing for adult men living with mental health challenges who are either:

- Moving from a higher support setting, so that setting can accommodate someone who is directly exiting homelessness that requires a high support setting; or
- Directly exiting homelessness themselves

Each tenant has his own bedroom (with an en-suite bathroom) and shares communal kitchen and lounge with other tenants.

This service is intended to provide affordable housing to people who are further along with their recovery journey and who are assessed as having the ability to live independently with only a mild level of support from Cota, delivered through prescheduled weekly appoints.

Referrals are internal to current clients enrolled and receiving support from one of our four Integrated Service Teams, while working closely with Coordinated Access and Access Point to determine eligibility.

**Primary Responsibilities**:
The Supportive Housing Worker is responsible for providing Recovery Oriented services to Cota service users in a manner consistent with Cota's Mission, Vision and Values.

The Supportive Housing Worker's primary responsibilities may involve but not necessary be limited to:
1) In conjunction with the manager and supervisor, the Supportive Housing Worker assists with the search for and acquisition of appropriate housing units as may be required within the program

2) Coordinates intake processes including:

- Reviewing the referral information of applicants to the program
- Coordinating and participating in screening interviews with applicants and their respective referral sources
- Determining eligibility for the program and confirming acceptance of new service users into the program
- Coordinating move-in arrangements with the new service users and their respective referral sources
- Facilitating internal move-in practices/protocols with new service users, per applicable program standards/guidelines
- Orientating new service users to the housing environment and Cota's service

3) Assists with the development of tenant Support/Goal Plans:

- Plays an active lead role in ensuring that Support/Goal Plans are developed with each tenant and his/her support network and a record of this is documented in service user's chart
- Shares responsibility with the Team for implementing appropriate housing supports that assist service users in pursuing/achieving their goals
- Liaises with service user support networks on a regular basis
- Focuses on assisting service users with:

- Tenancy Related Matters, such as:

- rent payments
- landlord disputes
- reducing high risk behaviours
- coping within a communal environment
- engagement within the life of the tenant community
- Activities of Daily Living (ADLs), such as:

- Maintaining appropriate medications routines
- Providing assistance and reminders relating to appointments
- Establishing and/or maintaining service users' personal hygiene routines
- Assisting with service users' laundry routines
- Maintaining and/or cleaning service users' units/rooms
- Assisting service users with menu planning, grocery shopping and/or cooking
- Managing and supporting service users with behavioural needs
- Other supports with ADLs as identified by the Supervisor and/or Team
- Other individual support requirements including, but not limited to:

- community orientation
- linking to Case Management and/or other services
