**PSW BEHAVIOURAL SUPPORT LEAD -Full-Time - SHELBURNE LONG TERM CARE**
**PSW Behavioural Support Lead **will serve as an in-house expert on identification and management of responsive behaviors. This dedicated lead will function in a specialty role providing behavioural support expertise to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.
The lead will work as a member long-term care home, and provide build the capacity of all staff within the home to support those residents with responsive behaviours by:
- Providing point of care education on successful interventions for providing care to residents
- Providing a structure for internal support around managing responsive/expressive behaviours
- Promoting and sustaining practices that support person-centered care
- Promoting a culture in the home that shifts from carrying out custodial tasks to a more integrated model of residents and families into directing care
- Promoting a “Consistency of care” approach
As a member of the Shelburne Long Term Care BSO Team, the individual will work in collaboration with the BSO team members from other long-term care homes, the community and the acute care sectors, other external supports (i.e. the Seniors Mental Health Outreach Team, NPSTAT, MH LHIN Home and Community Care, Mental Health and Addiction service providers, Peel Halton Acquired Brain Injury Services, primary care providers) and other health professionals as required.
The Lead will be the first/primary point of contact within the home when referrals for this resident population are made to external supports (i.e. the Seniors Mental Health Outreach Team, NPSTAT, MH LHIN Home and Community Care, Mental Health and Addiction service providers, Peel Halton Acquired Brain Injury Services, primary care providers) and other health professionals as required.
**This a full time 1.0 FTE position that operates as a part of the SEIU Local 1 Union.**
**Key Responsibilities and Duties**:
- Provide sound clinical knowledge and skills, act as clinical expert and resource, and provide comprehensive, quality client/family centered health care services
- Do site visits, as necessary, to assist in decision making for acceptance to the LTCH waitlist
- Develop a comprehensive transition plan with Care Community BSO partners to support the transition of a new or returning resident into the home;
- Aid in the transition of long-term care residents with responsive behaviours (new or existing) between community, hospital and neighboring long-term care homes (LTCHs).
Stemming from the transitional care plan, develop an ongoing care plan responsive to the needs of the resident related to responsive behaviours
- Provide/complete comprehensive bio-psychosocial geriatric assessments and or behavioural assessments, plan treatment strategies, follow-up and care planning in conjunction with team
- Collaborate with Psychogeriatric Resource consultants and mobile behaviour support resources (i.e. PRCs, NP Stat, System Navigator, Community Support Worker, etc.) as necessary to develop and implement evidence-based treatment/care plans responsive to the needs of residents
- Identify the need for additional consultation and/or referrals to external services or specialties and coordinate referrals
- Assist in the implementation and sustainability of care plans and recommendations of external resources by providing coaching to LTCH staff
- Identify and flag residents who require additional attention
- Provide support through the development and implementation of a home specific BSO strategy. Home Specific BSO strategy to be developed in collaboration with the management team of the home and the PRCs and could include the integration of volunteer services and therapeutic recreational programming.
- Identify the budgetary requirements for the implementation of the BSO strategy
- Identify risks and gaps within the home related to responsive behaviours and the education and training required to mitigate risk, create capacity, and build support
- As part of a home’s performance improvement plan, provide input to staff performance reviews as related to a staff member’s ability to care for clients with responsive behaviours. Participates in the development and delivery of educational and informational programs for residents and staff.
- Provides Education and Coaching to LTC home staff in the use of frameworks such as GPA & UFIRST to assess responsive behaviours,
- Provide coaching on the implementation of behaviour care plans and guide appropriate care plan transitions
- Facilitate transfer of knowledge into practice and enhance staff’s ability to manage responsive/expressive behaviours and care planning interventions through role modeling, coaching, and mentoring
