**Home Adaptation & Maintenance Program (HAMP) Coordinator**
Full-time, 12-month Contract with possibility for extension
Monday to Friday, 8:30 AM - 4:30 PM, 35 hours per week
Base office: Bradford
Reporting to HAMP Supervisor, you will be responsible for the coordination of functions within the HAMP program, with a focus on assessing and coordination of services for clients, assisting with program standardization and branding, relationship building and evaluation of outcome measures, while serving the client’s needs. To be a supplementary representative of the Home Adaptation & Maintenance Program with all health and community partners.
**What you bring to the role**:
- Experience with in-person client assessments.
- Senior Home Safety Specialist
- an asset.
- Demonstrated project management skills and high attention to detail.
- Exceptional communication and interpersonal skills.
- Excellent computer skills with proficiency in MS Word, Excel, and Outlook.
- Valid Ontario Driver's License and access to a vehicle.
- Flexibility to work outside normal business hours (8:30 AM to 4:30 PM) as required.
- Fluency in both official languages (English or French) or other language(s) an asset.
- Handyman/repair skills an asset.
