Position Overview

The Supervisor reports to the Manager, and is responsible for supervisory functions for Transition Navigators and any project work related to improving patient transitions. The supervisor will also be responsible for liaising with Community Supervisors to ensure safe and efficient patient transitions from hospital. The supervisor works with the MGH and community teams to ensure they are providing quality transition planning for patients returning to the community by ensuring they are being matched to the most appropriate community programs and services to meet their needs.

The Supervisor supports team members and works with community partners to coordinate and integrate services and will focus on on-going performance improvement. The incumbent supports the team by actively participating in community homecare projects such as MGH2Home and identifies other opportunities for engagement with community partners with a goal of reducing non acute LOS, reducing readmission rates and reducing overall ALC rates.

The Supervisor is the first point of contact for staff in relation to any escalations, internally or externally and offers support and assistance in trouble shooting issues related to discharge planning.

They will also be available to supportrecruitment and onboarding of new staff. The supervisor will provide vacation coverage for the Manager.

The incumbent contributes to corporate initiatives and quality Improvement projects to enhance Michael Garron Hospital’s mission, values and goals.

Education
- Undergraduate degree in a Clinical discipline.
- Current certificate of registration issued by the discipline's licensing body in Ontario.
- Minimum 3 years of recent experience working in an acute care Hospital.
- Minimum 3 Health System and Community Navigation experience.

Experience
- Demonstrated experience in incremental leadership responsibilities in Hospital Operations.
- Knowledge of patient/staff scheduling systems, payroll systems, workload management systems is preferred.
- Familiarity with improvement process principles is a definite asset.
- Demonstrates a process driven approach to problem solving.
- Demonstrated computer literacy with competence in Microsoft Word, Excel, PowerPoint, Outlook, Powerchart and Internet Explorer
- Flexible, organized andself-directed within scope of responsibility; takes initiative.
- Knowledge of process improvement methodologies and quality monitoring required.
- Excellent written and oral communication skills.
- Exceptional interpersonal skills.
- Demonstrated ability to de-escalate difficult situations.
