Nursing Home Without Walls Personal Support Worker

**Position Summary**

Kenneth E. Spencer Memorial Home, through its **Nursing Home Without Walls** initiative, is seeking a compassionate and community-focused **Personal Support Worker (PSW) / Activity Worker** to deliver personalized care and engagement services to seniors living independently at home.

This role is ideal for someone who values connection, dignity in care, and supporting older adults in maintaining their independence through a blend of personal care, companionship, and recreational engagement.

**Key Responsibilities**
- Provide direct personal care to clients including grooming, bathing, and assistance with activities of daily living (ADLs)
- Support the planning and facilitation of recreational, wellness, and social activities to promote client engagement
- Provide respite care and friendly visiting services for family caregivers
- Accompany clients to medical appointments and errands; support participation in community outings
- Assist in organizing, participating in, and cleaning up after client activities
- Conduct regular in-person or telephone wellness check-ins
- Fill gaps in home care service delivery to ensure continuity of care
- Provide bathing and related supports to clients accessing services at Kenneth E. Spencer Memorial Home
- Monitor and report changes in clients’ physical, emotional, or cognitive status to the appropriate team members
- Encourage and assist clients in maintaining social connections and independent routines
- Support clients with additional tasks as identified based on their needs
- Undertake additional responsibilities as assigned to support the program’s goals

**Qualifications & Experience**

**1. Education & Experience**
- Certificate in Personal Support Work or equivalent training in a healthcare-related field
- Minimum of 1-2 years of experience working with older adults in home, community, or long-term care settings

**2. Skills & Competencies**
- Knowledge of aging-related concerns including isolation, mobility, and chronic illness
- Strong interpersonal and communication skills
- Ability to work independently in a home-based or community setting
- Proficient with documenting observations and following care instructions
- Must have valid Driver's License and own vehicle

**Job Type**:
Full-time, Temporary - 6-month term with potential for extension

**Working Hours/Schedule**:
Mondays to Fridays with varied shifts including day, evening, and occasional weekend work as required, based on the needs of the clients
