**What we are looking for**:
As a Pharmacy Student at Oak Valley Health -Markham Site your primary role will include supporting the medication reconciliation process in various patient care areas of the hospital. Your duties will include (but are not limited to) collection and documentation of Best Possible Medication Histories (BPMHs). You will be trained by pharmacy team members, including pharmacy technicians and clinical pharmacists and will need to successfully complete a certification process. You will also be responsible to complete projects, as assigned by your preceptor or delegate.
**What you bring to the role**:
Currently a 3rd to 4th year student in a pharmacy program at university
Prior hospital experience is highly desirable
Well-developed organizational and prioritization skills; working in a fast-paced environment
Demonstrated effective communication and interpersonal skills with a patient-care focus
Sound independent work habits coupled with proven ability to work collaboratively as part of a team
Demonstrated good attendance and performance records with the ability to maintain these same standards
Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

**Work Schedule**:
This position will work 5 shifts per week, and will require day, evening and weekend flexibility
