Reporting to the Director, Seniors Services Development (SSD) the Manager will provide leadership and manage the integrated care centre to ensure quality outcomes, safe care, and effective and efficient delivery of services according to evidence-based best practices and relevant legislative requirements. In this role you will manage a team which includes both health-care and non-health-care professionals.

**What you will do in this role**:

- Responsible for ensuring the successful opening and operation of the integrated care centre, including but not limited to, the medical clinic, dental clinic, and respite centre
- Ensures that all clients receive individualized person-centred, emotion-based care for their physical and mental well-being while supporting their maximum independence and quality of life through the activities of interdisciplinary teams
- Monitors and evaluates the delivery of various administrative, paramedical, and contracted services to ensure quality of service delivery to meet client needs and safety. Identifies, develops, and implements other client specific services to meet client needs based on best practices for complex medical conditions and dementia care
- Responsible for the oversight and administration of the integrated care model including effective planning, organization, implementation, and evaluation
- Responsible for the collection and submission of program data and analytics requirements of funders such as, Ontario Health, provincial ministries and other continuous improvement initiatives
- Acts as an inspiring leader, creates an environment for collaborative teamwork and contributes to the development of a caring, compassionate, and competent team responsible for direct client care activities
- Monitors staff performance via ongoing audits of client care activities, observations, and documentation records
- Conducts performance management reviews with staff including goal setting, development of measures, assessment of achievement of goals, planning for the upcoming year, supporting the development of learning plans, etc. for all regulated, non-regulated, unionized, and non-unionized staff
- Collaborates with the SSD Director, Senior Medical Director SSD / Long Term Care (LTC)/ Community Paramedicine (CP), Adult Day Services (ADS) Medical Director, Community Support Services (CSS) Manager and ADS team, integrated care centre staff, Peel Manor Administrator, etc. related to clinical client care issues and professional practice, as needed
- Acts as a Supervisor under the Occupational Health & Safety Act (OHSA) responsible for worker safety, mental health, and psychological health and safety
- Develops, administers, and monitors a budget, which includes all salaries, materials & supplies to meet client needs and ensures staff have the tools/equipment/training to perform their duties

**What the role requires**:

- RN registration in good standing with the College of Nurses of Ontario along with completed Baccalaureate degree
- Five years of progressive leadership and management experience, including but not limited to supervisory experience in community-based programs for seniors, LTC, or health care or geriatric acute care
- Equivalent combination of education and experience may be considered
- Knowledge of health care delivery systems, changing health ecosystems, applicable legislations and regulations, quality assurance and both medical and non-medical needs of complex frail elderly clients living with dementia
- Strong understanding of _Ontario's Regulated Health Professions Act, 1991 (the 'RHPA'), the Personal Health Information Protection Act, 2004 (the 'PHIPA')_ and experience working with an integrated health team within the Circle of Care
