**SUMMARY**
Under the direction of the DWT IRSSP Program Coordinator, the Resolution Health Support Worker (RHSW) will provide and coordinate support services for former Indian Residential School (IRS) students and their family members. Working collaboratively with the Dakota Wicozani Tipi (DWT) Health Team, the RHSW will identify and assist former IRS students and their families, both urban and on-reserve, in accessing culturally safe, trauma-informed support services. These services will be respectful of and responsive to the diverse cultural practices and beliefs of the communities served, ensuring clients can safely address the impacts of their IRS experiences. This role includes serving members of the Dakota Nations and the Dakota Ojibway Tribal Council (DOTC) region.
**DUTIES AND RESPONSIBILITIES**
**Communications & Outreach**
- Increase awareness of the IRS Support Program through activities that align with DWT’s Vision, Principles, Values, and policies.
- Initiate and facilitate outreach activities to build strong community relationships and promote the DWT IRS Support Program (IRSSP).
- Develop regional and national relationships with other RHSWs to foster discussion, share best practices, and enhance support networks.
- Collaborate with other community-based programs to integrate and promote mutual services, ensuring seamless access and culturally appropriate delivery.
- Consult with external service providers (e.g., nurses, doctors, psychologists, educators) to develop integrated and holistic care plans.
**Case Management**
- Conduct comprehensive needs assessments to determine eligibility and support requirements of clients.
- Identify and recommend therapeutic and traditional healing supports; refer clients to Health Canada for counseling service approval.
- Maintain accurate case files including assessments, service plans, follow-up notes, statistics, and peer review documentation.
**Frontline Support Services**
- Provide trauma-informed frontline support to former IRS students and their families, including one-on-one and group-based approaches.
- Refer clients to local and regional resources (e.g., NNADAP, mental health programs, treatment centres).
- Offer services aligned with individual healing needs (e.g., dialogue, ceremonies, prayer, traditional healing).
- Monitor and document progress through regular client contact and case reviews.
**Administration & Reporting**
- Prepare and submit monthly, quarterly, and annual reports as required.
- Collaborate with the Health Director, Assistant Director, and Health Team on work plans, program evaluations, strategies, and policy updates.
- Develop tools to evaluate the effectiveness of the DWT IRSSP.
- Participate and provide reports in required meetings, workshops, training sessions, and conferences.
**Qualifications & Requirements**
- Degree or diploma in Social Work, Human Services, or a related field.
- Minimum of 3 years related experience.
- Strong understanding of First Nations cultural practices, traditions, and intergenerational trauma.
- Knowledge of the Indian Residential School system and Independent Assessment Process (IAP).
- Familiarity with federal/provincial health systems and Indigenous health frameworks is an asset.
- Certification in Mental Health First Aid or related training is an asset.
- Valid Emergency First Aid/CPR certification.
- Skilled in case management, integrated care planning, and trauma-informed approaches.
- Knowledge of wellness and healing resources (on/off reserve).
- Proven experience in group facilitation, public speaking, and conflict resolution.
- Strong written and verbal communication skills.
- Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams, Publisher).
- Ability to speak and understand Dakota (or Ojibway) is considered an asset.
- Valid Class 5 driver’s license, reliable vehicle, proof of insurance and willingness to travel.
- Ability to work flexible hours as needed.
- Must provide a current Criminal Record Check, Adult/Child Abuse Registry Check with Vulnerable Sector Check.
