Are you a caring and compassionate Personal Support Worker (PSW) looking to make a real difference? Join our **CBI Home Health’s Guelph-Wellington OHT Leading Practice** team, where we’re doing things in a new and exciting way As a PSW, you’ll work closely with a local primary care team to provide hands-on, person-centered care that helps people stay healthy and independent in their own homes. You’ll be part of a supportive home health team focused on preventing health problems before they happen, helping patients avoid trips to the hospital and live happier lives.

**What you will need to be successful**
- Your Personal Support Worker certificate or equivalent:

- Valid Certificate as Home Support Worker, I, II or III, Health Care Aid, Developmental Support Worker; Or
- Students currently enrolled (post first year) in health care programs such as Nursing (RN or RPN), Rehab, Para Medicine; Or
- A valid certificate as an RN, RPN, Practical Nurse, Midwife, or Physician (domestic or internationally trained)
- Current and valid Standard First Aid and CPR (Level C) Certification
- A clear Criminal Record Check with Vulnerable Sector Screening within the last 12 months
- Proof of a negative TB test within the last 12 months and an up-to-date immunization record
- Ability to travel from home to home within the community
- **a valid driver's license, vehicle and car insurance is required for this role**
- **Travel required within East Wellington rural communities such as Rockwood, Mapleton, Drayton & Mount Forest**
- Commitment to work every other weekend
- Strong verbal and written English communication skills required; the ability to communicate in French or other languages is considered an asset
- Reliability and strong organization skills
- Ability to safely lift up to 35 lbs. (16 kg)
