**Position Summary**
The Youth Transition Worker plays a pivotal role in supporting youth aged 15 to 18 years who are navigating the transition to independence. This position involves working collaboratively with community agencies to provide therapeutic support, life skills coaching, and assistance in securing permanent housing. The goal is to promote the emotional and social well-being of youth, helping them build a foundation for a successful future.
**Hours**: This is a full-time position with a 40-hour work week, Monday-Friday, with flexible hours. One year maternity leave coverage.
**Key Responsibilities**
- Develop and implement individualized transition plans in collaboration with youth, their families, and community partners.
- Provide one-on-one life skills mentoring, including education on age-appropriate behaviours and coping strategies.
- Assist youth in exploring and accessing community resources, including housing, education, and employment opportunities.
- Coordinate placements, intakes, and discharges, ensuring smooth transitions for youth entering or exiting programs.
- Train and support house managers to ensure consistent and effective support for youth.
- Respond to crises and provide conflict resolution as needed.
- Maintain regular communication with case management teams, providing updates and participating in case conferences.
- Manage and report on budgetary resources allocated for youth support services.
**Qualifications**
- ** Education & Experience**:
- College diploma with a minimum of three years' experience in youth work, or
- High school diploma with a minimum of seven years' experience in youth work.
- ** Certifications**:
- Valid First Aid and CPR certification.
- Crisis Prevention Intervention (CPI) certification.
- ** Other Requirements**:
- Valid B.C. Class 5 driver's license and access to a personal vehicle.
- Clear Criminal Record Check.
- Strong verbal and written communication skills.
- Knowledge of systems theory, counselling, advocacy, community networking, and life skills development.
- Ability to work collaboratively within a team and with external community resources.
