**Home Care Coordinator**

We looking for an enthusiastic HCA to join our team. Ideally suited for a health care-aid with 5-10 years of experience that would like to transition into an administrative and/or supervisory role while still providing care as needed.

**Responsibilities/Activities**
- Ensure Agency policies and procedures and industry standards and regulations are followed
- Assist with the coordination, evaluation and planning of home care services.
- Assist the Manager with administrative activities and optimal use of resources.
- Develop, implement and evaluate strategic plans, goals and objectives.
- Coordinate staffing activities.
- Coordinate scheduling activities.
- Once full-time in office, fill-in shifts as needed due to last-minute cancellations
- Review Care Notes daily
- Verify Visits daily
- Send out open shifts as needed
- Send out cancelled shifts on timely basis
- Fill shifts as company grows to a point of only filling shifts as a last resort
- Supervise work activities of designated employees.
- Quarterly caregiver check-ins in client’s home
- Develop and instruct employees in the use of practices, procedures and equipment.
- Provide leadership to employees.
- Conduct in-home assessments for new clients
- Create care plans including ADL confirmation and instructions
- Liaise with community resources and other agencies.

**Required Knowledge**
- Knowledge of standards of practice and scope of practice, in accordance with provincial regulations
- Knowledge of personal care and home management skills.
- Knowledge of principles and processes for providing client and personal care services, including needs determinants, meeting quality standards and evaluation of client satisfaction.
- Knowledge of the information and techniques needed to diagnose and treat injuries including emergency first aid and CPR.
- Knowledge of clerical procedures such as maintaining records and completing forms.

**Physical and Mental Demands**:

**Qualifications/Education**
- Health Care Aid (HCA) 5 + years
- Current driver’s license.
- Proper Vehicle Insurance Coverage.
- Willingness to obtain Vulnerable Sector Check
- First Aid/CPR

**Job Type**: Part-time

Pay: $24.34-$27.12 per hour
