**Position Overview**

The Quality Improvement Coordinator shall provide ongoing support to the management team and departmental staff related to Quality Improvement Initiatives, electronic documentation, and data management systems. Including but not limited to, auditing, training, data collection, evaluation, and risk management.

**Duties and Responsibilities**

Quality Improvement (QI):

- Act as a resource to management and departmental staff in relation to QI
- Provide in-services related to quality and risk indicators
- Provide quality indicator reports as required
- Attend/Facilitate meetings and assist with data interpretation
- Conduct audits for completeness and accuracy
- Attend education sessions as required
- Provide data reports to the management team which identify potential and actual quality and risk management issues
- Promote continuous quality improvement within the Home

Electronic Documentation:

- Develop Elder assessment schedule
- Ensure MDS assessments, Elder assessment protocols (RAPs) and care plans are completed accurately according to Elder assessment schedule
- Generate reports for use in clinical and operational decision-making
- Maintain current knowledge of system and act as a resource to users
- Provide training to new and existing staff as required

Data Management:

- Ensure the Elder assessment schedule is completed according to CIHI deadlines, and is accessible to RAI users
- Audit/Monitor assessments, RAPs and care plans and provide support and training to RAI users as needed
- Generate RAI-MDS reports as required for use in clinical and operational decision-making
- Attend education sessions, meetings and training sessions as required
- Provide training to new and existing staff as required
- Maintain contact with the Health Services Data Base of CCRS (CIHI) to stay current on changes to coding guidelines, bulletins, new RAI-MDS criteria and definitions, and share new information to users and management
- Maintain current knowledge of RAI-MDS and related New Ministry initiatives and share with management team
- Coordinate and provide guidance to contract users, i.e., Dietician, Physiotherapist to provide support in completing their designated MDS assessments, RAPs and care plans
- Provide RUG reports of each home area, ensuring data accuracy, and provide support and guidance to determine if funding is being maximized

General:

- Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed
- Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis

**Qualifications**

Minimum Formal Education
- Current regulated or licensed health care practitioner
- Completion of MDS credentialing program

Experience
- A minimum of three years clinical, project leadership or supervisory experience
- Proficiency in Microsoft Windows environment including Microsoft Outlook, Word, Excel and Point Click Care
- Demonstrated proficiency in RAI-MDS
- Knowledge and understanding of the principles and methodology of Continuous Quality Improvement
