One Rehab Inc. is a Hamilton-based company looking to build a diverse team of dedicated Rehabilitation Support Workers throughout Southern Ontario. At the forefront of emerging support services, we specialize in providing personalized care to assist our clients with achieving their optimal level of independence. Working under the direction of one or more healthcare professionals, the RSW provides support services to clients living with the effects of an acquired brain injury, mental health issues, spinal cord injuries and/or various complex needs. We provide in-home, community and remote care to support all our clients needs. Programs that may be implemented include life skills retraining, behavioural management, cognitive remediation, community reintegration, educational and vocational support. **Duties and Responsibilities**:
- Support adults with Acquired Brain Injury/chronic pain syndrome with activities of daily living, personal care, and rehabilitation planning.
- Support clients virtually to ensure their optimum physical, social and emotional well-being by connecting with them via text, audio call, and video call.
- Provide listening ear, emotional support, coaching on tools and strategies to cope with disability and complete daily activities.
- Offer support services as per company protocol to all clients as applicable.
- Support clients with their rehabilitation programs, as directed by regulated health care professionals, to help them reach their rehabilitation goals following a brain injury, work-related illness or accident, mental health diagnosis, or spinal cord injury.
- Assist in monitoring behaviors or activities that affect the safety, security, and well-being of clients and caregivers.
- Work cooperatively with clients, treatment teams to achieve specific objective and measurable goals
- Observe and report changes in the client’s health status or needs.
- Provide individualized customer service of the highest quality.
- Be respectful, courteous, and pleasant always.
- Well-developed interpersonal, critical thinking, problem solving and decision-making skills to resolve client care issues.
- Communicates effectively with clients, families, staff, and management.
- Maintain high-quality, accurate documentation of observations related client’s behavioral and cognitive limitations.
- Role modeling, cueing, and prompting effective communication skills.
- Work co-operatively with team members.
- Provide services on a flexible basis including evenings and weekends.
- Receive direction from management and implement regular feedback provided.
- Participate in quality improvement and continuous growth and training activities as directed.
- Adhere to workplace policies and operational procedures.
- Maintain attendance and punctuality within designated shift.
- Perform other duties as assigned.
**Skills and Qualifications**:
- Post-Secondary Diploma/ University Degree and/or Certificate from a recognized college are welcome.
- Experience working as a Rehab Support Worker with individuals with physical, cognitive and/or psychological impairments
- Minimum 2 years’ experience working directly with people with acquired brain injuries and/or complex injuries
- Flexible and collaborative work style
- Enjoy community-based work, including physical activities
- Ability to multitask and prioritize Mental Health Diagnosis
- Sound behavioral management skills, cognitive problem-solving skills.
- Excellent communication (written and verbal), time management and documentation skills.
- Computer and data entry skills.
- Effective problem solving and decision-making skills.
- Proven consistency in work performance and teamwork, including reliability and flexibility
- Experience in writing professional reports
- Bachelor’s degree in a related field (e.g., psychology, kinesiology, etc.) or a Diploma in occupational therapy, physiotherapy assistant, Recreation and Leisure
- Flexible and collaborative work style
- Enjoy community-based work, including physical activities
- Minimum of 1 years of experience working in a related field
- Proficiency in written and oral English communication
- Proficiency in using computer programs and internet-based services
- Relevant, updated resume
- Valid Driver's License, access to a vehicle and insurance to cover transporting clients
**Additional attributes considered an asset**:
- Valid CPR and First Aid certificates
- Knowledge of Psychological aspects of rehabilitation
