**WellFort is seeking Dietitian to join our team**
**Dietitian - Multiple roles (contract and permanent)**
The Dietitian develops and provides nutrition education programs for individuals, families, groups and communities. The Dietician works as a part of the inter-disciplinary team functioning both independently and with other staff in clinical and outreach activities.
While the proportion of time allocated to different functions may vary according to service needs, the Dietitian’s role encompasses a combination of direct care, health promotion and education, advocacy, community development, program development and evaluation.
**RESPONSIBILITIES**
Direct Care
Provides holistic care and acts as an entry point into the broader health or social service system including:
- Assess client's nutrition status by gaining an understanding of food habits or preferences, socio-economic and cultural background, psychological factors and clinical profile.
- Develop, implement and evaluate individual nutrition care plans according to the parameters of client's health, literacy level, income and relevant cultural and psychosocial factors
- Ensure continuity of care by maintaining up-to-date medical records, monitoring clients on a regular basis, and participating in case conferences with other members of health care team, as required
- Incorporate a self-management approach in individual counselling sessions, thus working collaboratively with clients to work towards client identified goals.
- Make appropriate referrals to other health professionals and community services and resources and complete appropriate documentation.
- Participating in the development, implementation, monitoring and evaluation of treatment, education, counseling, and health promotion services for individuals, families, and the community.
- Providing services to clients at outreach locations. Maintains contact as required by Clinical Protocols with the physician or other staff through telephone consultation.
Health Promotion and Education
- Support and facilitate local community initiatives to identify barriers and improve food access.
- Develop and maintain networks with community organizations and groups in order to develop collaborative initiatives.
- Act as a technical advisor on food and nutrition programs and issues to staff of WellFort and as a consultant to other local health and social service agencies.
- Develop, implement and evaluate education projects and activities to meet nutritional needs of groups in the community.
- Use local media to promote healthy eating behaviours, and raise local food issues.
- Identify the needs for nutrition education resources and obtain or develop relevant and current resources to meet these needs.
- Advocate with other health agencies to develop culturally relevant nutrition resources.
- Maintain professional competence through participation in professional development programs, networking with peers and other related health professionals
Advocacy and Community Development
- Support the advocacy efforts of clients, agencies and policy makers to reduce barriers for clients and the broader community.
- Works with clients, especially those who traditionally experience barriers, to access to ensure that they are treated with fairness and dignity.
- Engages with the community to enhance its ability to function independently, to improve or protect the environment, and to support the development of healthy public policy and practice.
- Participates in community projects or WellFort activities to provide education, information or to develop collaborative strategies.
Administrative and Other
- Maintains clear, accurate and timely written and electronic documentation including EMR, requisitions, reports, encounters and other information as required by the health centre, MOH, other regulatory bodies.
- Stays current on evidence-based strategies that improve health.
- Maintains an up-to-date knowledge of government guidelines, acts and legislation affecting client care.
- Participate in policy development and advocacy within WellFort and at the community level
- Supervise, support or provide education for professionals in training, contract staff, or students as requested
- Follows WellFort policies and protocol for administrative requests (ie. expenses, vacation).
- Function effectively within the various teams at WellFort and work professionally, collaboratively and respectfully with all colleagues in the Centre.
- Participates in quality assurance programs, chart audits and peer audit process as required.
- Participates in research when opportunities arise.
- Other duties and responsibilities as may be assigned from time to time.
**QUALIFICATION, EXPERIENCE & SKILLS**
- Undergraduate degree in food and nutrition from a recognized university.
