The Mental Health Case Manager’s role is to manage a caseload of Jewish seniors diagnosed with a mental illness and provide and coordinate psychosocial and concrete services to them with the goal of enhancing their functioning, autonomy and social integration.

**Duties**
- Psychosocial and/or financial assessment of clients
- Development and implementation of intervention plan in consultation with client,

family as well as other professionals or community partners
- Provide ongoing psychosocial support, crisis intervention, follow-up, monitoring and

reassessment of client’s needs, advocacy, and accompaniment to various client
appointments
- Regular home visits to client
- Group facilitation
- Maintaining ongoing communication with Homecare team and staff of the agency
- Consultation, collaboration, and advocacy with relevant internal and external partners
- Identification of needs for new service responses for this specific population
- Participation in training and educational workshops and keep up to date with the most

current trends related to seniors and Mental Health* Fulfillment of all administrative and data requirements

**Education, Knowledge and Skills**

Requirements:* Minimum Bachelor degree in Social Work
- Minimum of 2 years of experience working in the field of mental health
- Knowledge of the Jewish Community
- Solid base of clinical knowledge and skills
- Excellent listening and communication skills
- Ability to work under pressure and prioritize needs
- Ability to work in team
- Facility in use of Microsoft Office (Excel, Word, Outlook, PowerPoint)
