Maternity Leave

The family support counsellor will be responsible for facilitating client support groups and client education. They will collaborate, cooperate, and network with community agencies, including Home and Community Support, memory clinics, and other community programs. Further they will monitor and provide proactive follow-up for clients and care partners to ensure ongoing support and education needs.

They will be responsible for maintaining confidential, accurate and current client records.

**Qualifications**:

- University degree in Social Work, or Social Services Workerdiploma or University degree in gerontology, psychology, or related healthcare discipline
- Knowledge and work experience in the gerontology field, specifically Alzheimer’s disease and related disorders
- Minimum 2 years’ experience in providing education and supportive counseling to individuals and families affected by Alzheimer’s disease and other dementias
- Ability to lead, work independently and as a cooperative team member
- Excellent communication skills
- Experience in facilitation of groups
- Competency in computer skills including ability to use electronic communication devices and software pertaining to documentation.
- Flexibility in adapting to change
- Driver’s license and access to a reliable vehicle is required.
