**Position Description**

**Position Summary-**

Support Home Needed (Individual resides in your home every day)

Support home for a female Individual. Individual is quiet and easy going and needs support with some daily living skills, such as cooking, grocery shopping, money skills. Individual attends a day program a few times a week and enjoys arts and crafts, movies, baking (supported) and simple games. Individual is currently working on literacy skills. Needs support navigating home safety, as well as safe relationships.

Support home provider would have access to respite hours to be utilized (2 days/month) other than that the Individual will be in the support home.

Pay is on a monthly basis at a rate of $1867.70/month

Reporting to the Program Coordinator the Support Home Contractor is responsible for providing support to Individuals in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The Respite Contractor provides support that facilitates the Individual’s growth, development and achievement of personal goals.

The Support Home Contractor works closely and in consultation with the Service Coordinator and Program Coordinator and the Individual’s support team.

**1. Specific Duties and Responsibilities**

**Individual Support**
- Promote the interdependence of the Individual.
- Act as a role model, displaying professional attitudes and behaviour.
- Encourage a developmental approach with Individuals in an age and situation appropriate manner.
- Encourage community involvement for each Individual, providing opportunities for structured and informal involvement.
- Provide appropriate support to each Individual, adhering to ethical standards outlined in policy.
- Promote the Individual’s health and well-being, e.g., exercise, healthy meal choices, management of stress.
- Promote safety and health in all environments
- Relay important concerns and information to the Service Coordinator..
- Complete and submit monthly, or more often upon the request of the Service Coordinator written program updates regarding the Individual’s progress.
- Represent themselves, the program, and the association professionally and positively within the community as a whole.
- Present Individuals in a positive, respectful manner, maintaining confidentiality.
- Utilize community resources to promote a positive service profile and to research opportunities within the community.
- Participate in an effective working relationship with the community to promote and facilitate Individuals obtaining recreation or leisure

**Home and Community Support**
- Monitor and/or administer medications, as required.
- Follow daily routines in a respectful manner, ensuring Individuals are as involved as much as possible.
- Support the Individual in making safe and healthy choices

**Documentation**
- Complete, and keep confidential all documented records and files as required
- Keep all documentation safe and secure.

**Teamwork**
- Work effectively with team members and guardians in a positive and professional manner.

**Liaison**
- Establish and maintain effective and professional relationships with the Individual’s personal supports, including the individual’s guardian, family members, and natural community connections.

**Professional Development**
- Support Home contractor is responsible to pay for and Maintain all required professional development required for a Support Home.

**Qualifications**

Education
- High School Diploma

**or**
- Relevant Certificate or Diploma in Human Services

Other
- Criminal Record Check
- Vulnerable Sector Check
- Valid Alberta driver’s license and access to a safe vehicle.

**Note**: Equivalencies of education and experience accepted.

- 24hrs a day

**Job Type**: Fixed term contract
Contract length: 12 months

Pay: $1,867.70 per month
