**Job Overview**

**Duties**
- Conduct thorough patient examinations and assessments to diagnose medical conditions.
- Develop and implement individualized treatment plans based on patient needs.
- Provide ongoing management of chronic illnesses and acute care situations.
- Collaborate with specialists and other healthcare professionals to coordinate patient care.
- Maintain accurate and detailed medical records using electronic health record systems such as Cerner or Epic.
- Engage in data collection for research purposes to improve patient outcomes.
- Educate patients on health maintenance, disease prevention, and treatment options.
- Manage pain effectively through appropriate interventions and referrals when necessary.
- Provide home care services as needed, ensuring continuity of care for patients in their residences.
- Stay updated with advancements in medical practices, particularly in pediatrics and acute care settings.

**Skills**
- Strong knowledge of home care practices and protocols.
- Proficient in using electronic health record systems, specifically Cerner and Epic.
- Excellent research skills for staying informed about the latest medical developments.
- Ability to perform effectively in hospital settings as well as medical offices.
- Strong data collection skills for accurate patient assessment and treatment planning.
- Proficient in pain management techniques tailored to individual patient needs.
- Experience or interest in pediatrics is a plus, enhancing the scope of patient care provided.
- Exceptional communication skills to foster strong relationships with patients and their families.
- Ability to work collaboratively within a healthcare team while maintaining a focus on quality patient care.

Pay: $15,000.00-$40,000.00 per month
