**Job Overview**
We are seeking a compassionate and dedicated Health Care Aide to join our team. In this role, you will provide essential support to patients in various care settings, including assisted living facilities, nursing homes, and home health environments. Your primary focus will be on enhancing the quality of life for individuals by assisting them with daily activities and ensuring their comfort and safety.

**Duties**
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and meal preparation.
- Monitor and document vital signs, ensuring accurate medical records are maintained.
- Provide personal care services such as catheter care and assistance with mobility using equipment like Hoyer lifts.
- Support patients with Alzheimer’s and dementia care, focusing on memory care strategies to enhance cognitive function.
- Administer medications as prescribed while adhering to HIPAA regulations and maintaining patient confidentiality.
- Engage in patient observation to identify changes in condition or behavior and report findings to nursing staff.
- Maintain a clean and safe environment by performing light housekeeping duties, including cleaning and laundry tasks.
- Collaborate with healthcare professionals to develop individualized care plans that meet the needs of each patient.
- Provide companionship and emotional support to patients, fostering a positive atmosphere for healing and comfort.

**Requirements**:

- Previous experience in caregiving or health care aide roles is preferred, particularly in long-term care settings or home health environments.
- Knowledge of medical terminology and familiarity with EMR/EHR systems is advantageous.
- Certification in CPR is required; additional training in hospice care or working with developmental disabilities is a plus.
- Strong communication skills and the ability to work effectively with diverse populations, including seniors and individuals with disabilities.
- A compassionate demeanor combined with patience and empathy towards patients’ needs.
- Ability to perform physical tasks associated with patient care, including lifting and transferring patients safely.
