**Location: This is a remote-based position. Individuals can reside in the following locations: Work-From-Home with touchdown workstations available at Ottawa, Kingston, Pembroke clinic locations.**

**Status**: Full-Time

**Our Rehabilitation Case Management department is growing, and we have an** **immediate opening for a Rehabilitation Case Manager within Modern OT, a division of Lifemark.**

As a **Rehabilitation Case Manager,** you will be responsible for case management and coordination of matters related to both occupational and non-occupational illness and/or injury. As part of your role, you will be working closely with a rehabilitation team, insurance adjusters, lawyers, and medical professionals to obtain and review objective functional details to implement a successful and sustainable rehabilitation plan.

**Duties and Responsibilities**:

- Coordinate rehabilitation for individuals with serious injuries sustained in a motor vehicle accident
- Assess and determine needs and facilitate relevant referrals such as occupational therapy, physiotherapy, psychology, speech language pathology
- Explain rehabilitation and insurance processes to client, family members, and other stakeholders
- Attend medical appointments to support client
- Responsible for scheduling and maintaining regular, multidisciplinary team communication
- Provide verbal and written reports to stakeholders, including insurance adjusters, lawyers
- Monitor and continually evaluate progress in rehabilitation and troubleshoot as necessary

**Qualifications**:

- Superior communication, listening skills, and comfort with preparing detailed written reports.
- Familiarity with reading medical documentation and with medical terminology
- Extremely organized, excellent time management, able to multitask, and take initiative
- Highly attentive to attention to detail
- Ability to recognize and understand differing views, and prioritize competing needs / recommendations
- Ability to work collaboratively with a team of professionals, to take a lead role in meetings
- Ability to identify issues and set goals within a group
- Dedicated and able to make tough decisions within tight time constraints.
- Tech savvy, with superior computer skills including with Office 365, Teams
- Excellent documentation and time management skills, able to meet/exceed required turn-around times
- Related professional designation such as College of Vocational Rehabilitation Professionals (CVRP) is an asset
- Bilingualism preferred
