**JOB TITLE**: Care Coordinator
**LOCATION**: Onsite, CUPS - 1001 10 Ave SW, Calgary,
**REPORTS TO**: Team Lead, Care Coordination and Intake
**SCHEDULE**: Full-time, Mon to Fri, 8:00 am-4:00pm
**CONTRACT TYPE**: Term Role to December 2026
**YOUR IMPACT**
The Care Coordinator is responsible for co-creating a caring, dignified and trauma-informed experience for all guests and clients at CUPS. The Care Coordinator will provide guests with holistic information regarding CUPS services for clients to make informed decisions regarding the supports of interest to them. Care Coordinators will work in partnership with CUPS clients to develop care plans that are sustainable and relevant to the client, linking the client to appropriate supports both within CUPS services and externally.
**WHAT YOU’LL DO**
**Client-Centered Support**
- As an initial contact position for all clients of CUPS, the Care Coordinator will ensure clients receive respectful, trauma-informed care by assisting the client where they are at providing information, de-escalation support, and fulfilling crisis support as appropriate and available.
- Operate in synergy with CUPS internal departments continuously assessing client needs, gaps in service, care plan modifications and advocating for the voice and self-determination of the client
- Assist clients in defining initial strengths and needs through CUPS standardized assessment and track progress with the client via follow up assessments to celebrate achievements and determine further needs
- In collaboration with the client, develop a care plan to meet the needs identified and connect the client to services internally and externally to achieve goals
**Collaborative, Trauma-Informed Care**
- Work closely with a multidisciplinary team, including healthcare professionals, social workers, and external service providers, to ensure seamless client support.
- Assist with crisis intervention and de-escalation when necessary, helping clients regulate emotions and access services effectively.
- Remain up to date on internal changes to programs, services and personnel to ensure accurate care plan development and warm hand-offs to colleagues
- Advocate for clients, ensuring their voice and choices are honored in service planning and delivery.
**Data & Reporting**
- Accurately document client interactions, care plans, and interventions in organizational and external databases.
- Track and report on client progress and outcomes to support program evaluation and continuous improvement.
**Work Environment**
- Combination of individual and team based work
- Times of extended hours of sitting
- Periods of high stress during busy periods
- Exposure to escalated guests, traumatic incidents and vicarious trauma
**WHAT YOU BRING**
- **Education & Experience**:
- Bachelor’s Degree in Social Services (Social Work, Psychology) with 2-3 years of experience OR
- Diploma in Human Services with 3-5 years of experience in the vulnerable sector.
**Knowledge & Skills**:
- Strong understanding of Calgary’s social services landscape, including government and non-profit supports. Specific experience of Calgary homeless sector an asset
- Skilled in engagement with vulnerable clients, crisis intervention and de-escalation strategies.
- Experience with client assessment tools and service outcome tracking a benefit
- Proficiency in client databases (e.g., FCSS, HMIS) considered an asset.
- Demonstrated ability to work effectively within a multidisciplinary team.
- Commitment to trauma-informed, client-centered care.
