**GENERAL ACCOUNTABILITY**:
The CPH Assistant Home Support Leader is directly accountable to their CPH Home Support Leader and their Home Support Coordinator and indirectly to the Associate & Executive Director. He/she shall be directly accountable for providing high quality support to people, working together with their support team and Home Support Leader to ensure that goals and objectives are met. Specifically, the Assistant Home Support Leader is responsible to:

- Provide direct support in a manner that is consistent with CPH philosophy and personalized support plans.
- Follow through on team decisions, support plans and directions from their Home Supports Leader and/or Home Supports Coordinator.
- Participate in the development, support and delivery of personalized plans for the people they support, including attending meetings as requested.
- Develop strong, positive relationships with the people they support, their families, co-supports and external agencies involved with the people CPH serves.
- Participate in Person Centered Planning for people receiving support.
- Attend training, team meetings, etc.as requested/required
- Actively participates in biannual Leadership Meetings
- Provide assistance to the Home Support Leader in tasks as assigned.
- Is the first person on call for the home during approved Home Support Leader absences.
- Assisting to ensure that home is maintained in a clean and healthy manner.
- Maintains ongoing communication with the Home Support Leader & Home Support Coordinator.
- The ability to manage and respond to crisis situations that may arise

This position reports directly to the Senior Home Support Leader. The Assistant Home Support Leader is a full-time role. This role will be expected to support a minimum of 35 hours per week in the home and to complete administrative duties as required. Evaluations are completed at 3 months, 12 months and annually thereafter. Positive Performance evaluations at 12 months result in a role permanency, if available.

Good relations must be developed and maintained with the people we support, their network (family, friends, and personal networks), CPH management personnel and other co-supports at CPH. Good external relations must be developed and maintained with CLSD Social Workers, health care professionals, agencies and community employers.

The most important decisions of the CPH Assistant Home Supports Leader relate to quality of life and support provided to individuals particularly:

- Ensuring the support provided is of high quality, person centered, and in harmony with planning CPH philosophy and Gentle Teaching.
- Always working towards the individual’s overall satisfaction. The support person understands that he/she is coming into the home/life of the people served by CPH. The recognition of this must remain the foremost in our minds. We must therefore respect the wishes in their service provision within the parameters of safety and quality of life. Those parameters must be decided at the team level with the support of the person, the family/personal network, funding bodies and community supports (where available).
- Assuming responsibilities and decision making of the Home Supports Leader during approved Home Supports Leader absences, unexpected illness or events and Home Supports Leader annual holidays.

**PRINCIPLE FUNCTIONS AND ACCOUNTABILITY**:
1. Provides direct support and lifestyle planning consistent with CPH beliefs and values and Gentle Teaching.

2. Provides direct support to people relevant to their personal plan and contract expectations.

3. Provides direct support that reflects the Gentle Teaching Principles, in that:
a. Intent is not in changing others behavior but in changing ourselves

b. Support is always focused on teaching the person to feel safe and loved and become more loving and engaged.

c. Positive use of the 4 tools in everyday support (hands, eyes, words, presence).

4. Support to people may include:
a. Participating in ongoing evaluations of satisfaction of people served

b. Assisting people in problem solving or decision making

c. Providing assistance to people in completing daily tasks

d. Sharing and/or developing options for community involvement, employment or recreation.

e. Participating in Yearly Reviews of PCP and maintaining

familiarity with people’s plans, goals, and team decisions regarding support approaches.

f. Participating in team meetings and providing input, suggestions or ideas.

g. Liaising and cooperating with each person’s day program concerns individuals

h. As required is responsible for medication administration in a manner consistent with the CPH medication policies and procedures.

i. Liaising with health care professionals as needed regarding people’s needs.

j. Maintaining valid First Aid and CPR (Level C) certification and be willing to provide First Aid assistance if necessary.
