**Profile**

Reporting to the Regional Manager, Community Programs & After Stroke, the **Coordinator, After Stroke **focuses on delivering quality programs and services to those who have survived a stroke. They will serve stroke survivors, caregivers, and families in Newfoundland and Labrador with personalized support throughout the entire stroke recovery journey; ensuring the needs of stroke survivors and caregivers are identified and addressed. They will work collaboratively with a range of organizations, clinicians and other professionals to create meaningful networks, providing the best possible support throughout the After Stroke pathway and into the community. The Coordinator represents MODC throughout their region and as such plays an essential role in building relationships and identifying opportunities for new programming and expansion of existing MODC services.

**Please note, this is a full-time, one-year contract.**

**Responsibilities**:

- Organize, manage, facilitate, and deliver After Stroke programs and services (either in-person or virtual) that are educational and supportive.
- Assist stroke survivors, caregivers, and family to understand and identify their recovery needs and support them to prioritize and set SMART goals and develop personalized stroke recovery plans.
- Provide consistent system and community service navigation to stroke survivors, caregivers, and family, acting as an information resource and support system.
- Utilizes the Customer Relationship Management (CRM) system to capture and analyze client data, including client profile, needs assessment, goals, achievements, and intervention involvement.
- Manage program resources effectively to meet program goals and objectives in a changing environment.
- Under the direction of management, implement and monitor evaluation standards for the After Stroke program to demonstrate impact.
- Work with other programs of MODC to promote and support the work of the After Stroke program.
- Develops relationships and collaborates with regional healthcare sites and staff who discharge stroke survivors, to build direct non-clinical referral pathways.
- Stay up to date with new developments and trends in knowledge, treatment and services related to stroke
- Oversee special projects, as required

**Qualifications**:

- Post-secondary degree or diploma in health promotion, social work, education, recreation, community development, occupational therapy, or related area.
- 3-5 years related health and education experience
- Experience working with people directly affected by stroke (stroke survivors, caregivers and family)
- Experience working with the disability community, knowledge of disability issues and/or lived experience of disability will be considered strong assets
- Experience providing person centred support
- Experience working with health and social care professionals in a variety of settings
- Experience with group facilitation
- Intermediate computer skills including Microsoft Office, video conferencing software and other virtual platforms
- Standard First Aid and CPR certification an asset
