Temporary Full Time
An exciting Temporary Full-Time opportunity as a Social Worker exists in the Family Practice Health Team reporting to the reporting to the Executive Director Family Practice Health Centre.
As a key member of Women’s College Academic Family Health Team (WC FHT), and an integral part of an interdisciplinary health care team, you will deliver social worker care in accordance with policies and professional practice standards. You will perform routine social worker duties, and ensure that the social needs of our patients are met. Your primary roles are to work in collaboration with the interdisciplinary team to perform patient, family and couples psychosocial assessments, short-term counseling, education, and advocacy. Performing these roles well will reduce unnecessary office visits and visits to after-care clinics or emergency rooms. You will deliver services to a variety of patient populations.
**Summary of Duties, but not limited to**:
- Provide patients and families with emotional support, coping strategies and linkage to community resources and support, including but not limited to financial assessment, social and financial assistance, options for disability support, respite care, and child care.
- Provide support, short-term and longitudinal psychosocial therapy and counseling through stages of women’s treatment journey, from diagnosis to aftercare and survivorship.
- Conduct assessments and participate in the development of treatment plans with other team members.
- Coordinate referrals and facilitate access to other health care providers and organizations, while assisting patients and their families with the development of social and community support systems.
- Develop, implement and balance individual, group, same-day counselling and patient and family education sessions.
- Work collaboratively with all members of the health care team internal and external to the organization while ensuring continuity of care as patients transition across the continuum through appropriate discharge planning, patient education, and coordination of services to community support and resources.
- Establishes a caring and therapeutic relationship with patients; demonstrates trust, respect, honesty and provides counseling including crisis interventions as required
- Advocates for patients by mobilizing resources and identifying and addressing gaps in service to meet supportive care needs of patients
- In collaboration with Professional Practice, Research, and Education departments initiates and participates in clinical research and teaching, and integrates research into practice to ensure best practice standards are maintained.
- Identify and lead quality improvement initiatives that will advance the work of the Centre and WCH
- Assist patients and families in overcoming barriers within the health care system to ensure timely delivery of services.

**Qualifications/Skills**:
- Registration with the Ontario College of Social Workers and Social Service Workers is required
- Master’s degree in Social Work or Counselling is required
- Minimum of 5 years’ experience in a hospital, ambulatory setting is required
- Ability to form positive therapeutic alliance and to work independently and within a multidisciplinary team
- Understanding of adult education strategies, teaching and learning principles, and advanced therapeutic communication techniques
- Ability to develop collaborative working relationships with a wide variety of internal and external stakeholders including community organization
