**TEMPORARY PART-TIME (22.5 Hours Per Week) Until March, 2027 with the possibility of extension**

Located on the coast of **beautiful Georgian Bay**, the West Parry Sound Ontario Health Team (WPS OHT) offers the best of both nature and technology. Ontario Health Teams are Ministry-approved collaborative health services made up of provider organizations including primary care, hospice/palliative care, long-term care, home and community care, mental health and addiction services, acute care, and other community-based services. These teams are collectively dedicated to the equitable improvement of population health and finding better, more efficient ways to receive and deliver care and services.
**Primary Responsibilities**:
- Manage assigned projects through their full lifecycle, including initiatives related to primary care access and attachment, chronic disease management, integrated care pathways, and digital health.
- Develop and maintain key project documentation (e.g., charters, schedules, communications plans, project management tools, budgets, and evaluation tools).
- Facilitate cross-sectoral collaboration, including the meaningful engagement of individuals with lived and living experience.
- Support planning and development of new care pathways and integrated models of service.
- Provide support and coordination for the Primary Care Network and other advisory or working groups.
- Develop and implement internal evaluation and reporting systems that align project activities with OHT outcomes and metrics.
- Assist with collation of data to support evidence-informed decisions.
- Contribute to strategic planning and regional decision-making through regular progress updates, reports, and presentations.
- Foster and support a safe, respectful, and culturally appropriate work environment aligned with OHT policies and Indigenous cultural safety principles.
**Position Prerequisites**:
- University degree in health, public administration, community planning, or a related discipline (Master’s preferred; equivalent experience will be considered).
- Demonstrated experience managing projects in a health or social services context.
- Experience in implementation science, quality improvement (e.g., Lean, Six Sigma), risk management, or human factors is an asset.
- Familiarity with rural/remote healthcare challenges and Ontario Health Team models.
- Strong asset to be familiar with primary care models in Ontario
- Strong asset to have experience working with primary care EMR and data
- Strong stakeholder engagement skills across diverse roles and sectors.
- Experience working with data sets and digital tools to support decision-making.
- Proficient with Microsoft Office and virtual collaboration tools (e.g., Teams, Zoom).
- Demonstrated learning in Indigenous cultural safety and allyship, with preference for experience working with Indigenous communities.
**Knowledge & Skill**:
- Project Management Expertise: Demonstrated knowledge of project management principles, tools, and methodologies to lead initiatives from inception through completion, including scheduling, budgeting, risk assessment, and evaluation.
- Health Systems Knowledge: Understanding of Ontario’s health system transformation, particularly in primary care, chronic disease management, digital health, and integrated models of care.
- Collaborative Leadership: Proven ability to facilitate collaboration across diverse sectors, including health, social services, and Indigenous partners, and to engage individuals with lived and living experience in meaningful and respectful ways.
- Analytical and Evaluation Skills: Ability to design and implement evaluation frameworks, analyze data, and use evidence to guide decision-making and continuous improvement.
- Communication and Interpersonal Skills: Excellent verbal and written communication skills, including the ability to draft reports, deliver presentations, and communicate with a variety of stakeholders in a clear and engaging manner.
- Administrative and Coordination Abilities: Strong organizational skills to provide secretariat support for committees, manage project documentation, and maintain internal reporting systems.
- Cultural Competency: Knowledge of, and sensitivity to, culturally safe practices, with a demonstrated commitment to equity, diversity, inclusion, and reconciliation, particularly in Indigenous health contexts.
