**LOCATION**:Home office is Brockville with travel to Gananoque and Mallorytown as required
**REPORTS TO**:Executive Director
: In conjunction with the Executive Director, the Program Manager (PM) oversees the operations of the Upper Canada Family Health Team (UCFHT) Programs and ensures the goals and objectives for each program are met. Working with the Allied Health Staff (AHP) in each program, the PM is responsible for the creation of appropriate, measurable outcomes, tracking and reporting these to Ontario Health (OH) through quarterly and annual reports.
In collaboration with physician clinical leadership, the PM is responsible for ensuring appropriate clinical care, competency and performance of clinical staff are being delivered in each program.
**Responsibilities**:
- Lead and manage the allied health staff within the organization
- Ensure programs are in alignment with the needs of the patient population and support the FHO physician practices
- Ensure a high quality of care is provided to all patients
- Develop health and safety, quality, policies and procedure processes and ensure documentation
- Ensure effective management of clinical resources
- Provide leadership in development, implementation and evaluation of programs and supports/ supervises program staff in program delivery
- Monitor, manage and oversee the planning, coordination and administration of each program, ensuring adherence to the UCFHT standards, guidelines, policies and procedures
**Qualifications**:
- Post secondary education in business, health or a related discipline plus 3-5 years’ experience working in a FHT, FHO or other primary care setting managing staff and overseeing day to day operations
- Experience using Telus PSS EMR and/or Avaros EMR
- Experience in proposal development and reporting required by funding organizations an asset
- Excellent communication and public speaking skills
- Demonstrated teamwork, leadership and supervisory skills
- Excellent organizational, analytical and problem solving skills
- Ability to work independently and in a team environment
- Computer skills including excel, MS office
**Working Conditions**:
- Non-standard hours of work
- This position requires work outside of the office environment
- Use of a personal vehicle is required
- A valid driver’s license is required
