**Role Summary**
Reporting to the PCN Director, and under the guidance of the South Island First Nations Communities, the Indigenous PCN Manager will work closely with the PCN Team, community service organizations and other Indigenous service organizations involved in primary care in South Island.
The Indigenous PCN Manager will be responsible for providing an Indigenous and Diversity Equity and Inclusion (DEI) lens on all aspects of planning, organizing and change management activities to support the implementation of the South Island PCN.
The role will include engaging with partner organizations to achieve the shared goal of improving health outcomes for Indigenous Peoples and other underserved populations.
The Indigenous PCN Manager also oversees the implementation of approved PCN projects and initiatives in collaboration with South Island First Nations communities, Island Health staff, Patient Medical Home (PMH) programs/services, primary care providers (including Fee for Service Physicians), and community agencies/organizations.
**Duties and Responsibilities**:
**Leadership**
- Guided by knowledge from Local First Nation Communities, Métis Charter Community, Inuit community, Indigenous Urban and Away from Home, and Indigenous Service organizations, and in consultation with the PCN Indigenous committees.
- Leads and supports the implementation of PCN services for Indigenous community members residing in Tsartlip, Tsawout, Tseycum, Pauquachin, T’Sou-ke, Pacheedaht, Esquimalt, Scia’new and Songhees Nations.
- Incorporates Indigenous wisdom when developing and growing the PCN through drawing upon personal experiences and guidance from local Elders.
- Champions, demonstrates, and leads with cultural informed practices that foster the growth and development of the PCN at all levels.
- Leads the integration of DEI principles and practices into core organizational structures, policies, and practices to promote equitable access to care.
- Oversees implementation of primary care initiatives, goals, and objectives within the PCN which includes working with Patient Medical Home (PMH) programs/services, other primary care providers, Island Health, community agencies, and other partners.
- Recognizes the issues around attachment and improved health outcomes for Indigenous Peoples and collaborates on recommendations for sustainable improvements.
- Supports the mission, vision, values, policies, procedures, and strategic priorities of the South Island PCN.
- Ensures the inclusion of patient voices within and across the PCN.
- Actively participates in the effective governance and function of the PCN by providing administrative and proactive strategic support to the PCN Steering Committee and working groups, and by ensuring effective engagement of all partners and people impacted by the PCN.
**Program Management & Reporting**
- Manages approved PCN projects and initiatives.
- Works with available resources to support effective change management.
- Establishes operational policies and procedures to ensure consistent practice across the PCN. Develops and recommends capital and operating budget plans for services.
- Identifies clinical and organizational opportunities for analysis, investigation, and improvement. Contributes to reporting to the Ministry, Family Practice Services Committee, and other partners, as required.
- Supports quality improvement and evaluation activities; including access and quality of care for populations served by the PCN.
- Promotes PSP supports such as panel management and advance access training for providers.
**Engagement**
- Maintains strong relationships with local Indigenous communities by being present, learning from
- Elders, and ensuring consistent communication and follow through.
- Engages with local Indigenous partners to learn and model local traditions and protocols and to understand community-level needs.
