**Responsible to**: Director of Service Delivery

Responsible for the overall, implementation, monitoring and evaluation of person-centered plans. To develop supports that respect the choices and needs of service recipients. Holds primary responsibility for the support and supervision of the Service Delivery Program and staff.

**Responsibilities and Duties**:
**Service Delivery**

1. Responsible for ensuring the delivery of appropriate and effective person-centered programs and services.

2. Facilitates and ensures person-centred plans address life goals as well as development, behavioural, health, and social needs.

3. Evaluate and modify programs to ensure they meet the changing choices and needs of the individual.

4. In Conjunction with the Director of Service Delivery identify need for Comprehensive Behavour Support Plans (“CBSP”), and develop such in compliance with CPP&SP.

5. Work in conjunction with, and act as a resource for direct support staff to implement person-centered plans.

6. Practices and teaches supportive decision-making practices.

7. Work co-operatively with community partners and seek additional supports for service recipients as required.

8. Conduct service recipient transition planning and participates on admissions committee.

9. Support individuals demonstrating unpredictable behaviours, including aggression &/or violence.

10. Complete Service Recipient assessments and analysis.

11. To work frontline as required.

**Personnel**

1. Assist in the employment, orientation, training, and evaluation of program staff.

2. Supervise and work with staff to co-ordination and implementation of service recipient programs.

3. Provide training on service delivery to Direct Support Professionals and Supervisors and acts as a resource, particularly in the area of positive behaviour supports.

4. Supervise, support, and provide regular feedback and coaching to direct reports and front-line staff.

5. Interview and select direct report employees and volunteers.

6. Proactively deal with situations and implement corrective action in a manner consistent with Collective Agreement.

7. Ensure compliance with policies and procedures including safe work practices.

8. Establish and implement long and short-term goals for individual staff members based upon an assessment of their skills and abilities.

9. Report and/or refer any significant events, situations, discrepancies, and unethical behavior or procedures to the Director of Service Delivery.

**Administrative Tasks**

1. Co-ordinate Person Centered Plan meetings.

2. Implement strategies/formats for best practice person-centered plans.

3. Monitor the accurate record keeping of individual service recipient’s progress.

4. Maintain service recipient records and documentation ensuring confidentiality, accuracy, professionalism and timely documentation.

5. Complete correspondence and reports as required.

6. Support policy and procedure development.

7. Participate in meetings such as staff, program, OH&S, and other relevant committees.

**Management**

1. To meet, as requested, with Collective Bargaining Committee during negotiations or at other times for consideration of revisions to or other matters connected with such agreement.

2. Provide information to and advise the Director of Service Delivery on training and cost analysis with respect to the level of skills necessary when assessing new contracts.

3. Evaluate equipment needs and space requirements as well as an appropriate plan for present and future needs of the agency.

4. Other duties as assigned by the Director of Service Delivery.

**Required Knowledges, Abilities and Skills**

1. Ability to organize, evaluate, and co-ordinate programs.

2. Completion or willingness to complete CLSD Program Coordinator Level 3 training.

3. Knowledge of crisis prevention and intervention techniques.

4. Strong analytical and problem-solving skills.

5. Strong verbal and written communication.

6. Ability to facilitate meetings and training.

7. Positive and effective interpersonal skills.

9. Possesses an understanding of lifts and repositioning techniques, specialized care equipment, and policies and procedures related to the administration of medication.

10. Committed to lifelong learning and improvement in practice.

**Required Educational/Experience**

1. Degree/Certificate in Social Work, Diploma in Psychiatric Nursing or equivalent combination of education and experience.

2. Minimum 3 years of experience supporting adults with intellectual disabilities.

3. Minimum 2 years of supervisory experience.

4. Minimum 2 years’ experience in the design and implementation of person-centered programming and behaviour support plans.

**Other Requirements**

1. A valid Saskatchewan Driver’s license.

2. Must provide a Criminal Record Check with Vulnerable Sector Check prior to employment.

3. Be available to respond to emergency needs as required.

4. Current First Aid/CPR Certification
