DAY PROGRAM FACILITATOR for Adults with Special Needs

JOB DESCRIPTION & DUTIES

DESCRIPTION:
Under the direction of the Hellenic Hope Center for Persons with Special Needs Inc. Board of Directors and the Program Manager/Administrator, the Day Program Facilitator plans, leads and evaluates all Individual
Support Plans designed to meet the physical, intellectual, social and recreational needs of the participants and provides emotional support and personal assistance to participants with all special needs in order to ensure their optimal participation in the Hellenic Hope Center Day Program.

DUTIES:

- Implements support for participants to achieve personal goals as established through Individual

Support Plans during the annual parent conferences.
- Responsible for the monitoring and evaluation of services for each participant and maintains records of assigned files, including any reports and Individual Support Plans for each participant.
- Records and maintains appropriate information in daily schedule logs, individual files and related records.
- Communicates on a regular basis with the other Day Program Facilitator(s) and the Personal Support

Worker(s) regarding each participant’s Individual Support Plans.
- Participates in weekly meetings with all Day Program Staff.
- Plans, implements, monitors and evaluates Day Program activities. Activities are to be planned within the different developmental domains and will include life skills training.
- Organizes and participates in leisure activities (e.g. swimming, walking and bowling) in the community.
- Assists in arranging for transportation to all Day Program outings.
- Supports participants in daily activities by encouraging choices and independence.
- Researches and adapts activities to meet the needs of the participant’s interests and abilities.
- Leads or assists with implementation of daily activities.
- Ensures that activity areas and equipment are in a clean and safe condition and ensures the health and safety of all participants by performing general cleaning and maintenance.
- Assists with purchase and preparation of nutritious meals and snacks, including serving and clean up of kitchen.
- Ensures health, safety and welfare of all participants is supported.

Ensures that participants’ daily emotional and physical needs are addressed, including personal assistance.
- Assists participants with daily living activities as well as feeding, toileting, hygiene, showering and dressing/undressing while promoting independence.
- Observes, reports and documents any changes in a participants’ physical, emotional or psychological functioning.
- Supports each participant to develop skills and assume roles which promote his/her effective participation in the community.
- Deals with behavioural issues according to the HHC Policies and Procedures.
- Reports to the Ministry of Community and Social Services or the local Police Department immediately any allegations, or suspicion of abuse with respect to any participant or staff.
- Assists with the recruiting, training, placement and evaluation of Day Program volunteers and/or co
- op placement students.
- Participates in various meetings as directed by the Board of Directors.
- Develops a monthly schedule of activities and costs and submits it in advance to the Program

Manager for approval.
- Implements and maintains safety rules and regulations, including monthly health and safety inspections and drills to ensure safety compliance for the Day Program Participants and Staff.
- Takes all reasonable precautions for the safety of all participants, staff and volunteers.
- Ensures accurate incident/serious occurrence reporting is completed accurately by all parties involved and debriefs Programs Manager/Administrator and Parents along with any staff member involved.
- Maintains open communication with each participant’s parent/guardian and ensures confidentiality.

All discussions regarding the participant are conducted in private before or after the Day Program hours, unless it is an urgent issue.
- Initiates, coordinates and conducts annual parent conference to review and assess the participant’s

Individual Support Plan.
- Follows, refers to and abides by the HHC Policies and Procedures Manual.
- Performs all other related duties as assigned by the Board of Directors, Programs Committee and Programs Manager/Administrator.

**Salary**: From $20.00 per hour
