Are you a dynamic healthcare leader passionate about improving care transitions and enhancing patient outcomes? Join our team as **Manager, Transitional Programs** and lead the Hospital to Home (H2H) Program— ensuring safe, coordinated, and patient-centered transitions from hospital to home or other community-based care.

In this role, you will oversee interdisciplinary teams, foster partnerships across sectors, and drive quality improvements that reduce hospital readmissions, support recovery, and enhance the patient and caregiver experience.

**Key Responsibilities**
- Lead the planning, implementation, and evaluation of SJCCC’s Hospital to Home program across multiple sites.
- Serve as the primary liaison between SJCCC and contracted providers
- Provide leadership to transition team members, fostering a culture of collaboration, learning, and accountability.
- Ensure individualized, equitable, and goal-driven care transitions that address social determinants of health.
- Build strong partnerships with hospitals, primary care, and community providers to enhance system integration.
- Monitor contract performance, service delivery standards and adherence to key performance indicators such as missed care, readmissions, length of stay, patient satisfaction) and lead quality improvement initiatives.
- Manage program budget and resources effectively, driving operational excellence.

**Qualifications**
- Bachelor’s degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or related field required. Master’s degree in Health Administration, Public Health, or equivalent preferred.
- Minimum 5 years in progressive healthcare leadership, with expertise in home care, transitional care, or integrated care.
- Strong leadership, communication, and strategic thinking skills.
- Knowledge of care transitions, system navigation, and digital health tools is essential.
- Experience working with older adults, patients with complex health needs, and vulnerable populations is an asset.
- COVID 19 Vaccinations (2 Dosed) required to work in our facility.

**Job Types**: Full-time, Fixed term contract

Pay: $48.45-$60.56 per hour
