JOB TITLE: Clinical Case Manager
REPORTING TO: Program Manager
: The Clinical Case Manager manages a caseload to provide supports to youth in the Independent
Options/Young Parent Program, including conducting assessments to ensure program alignment with youths’ needs, developing comprehensive care plans and monitoring and adjusting them on an ongoing basis. The Clinical Case Manager focuses on developing and maintaining a therapeutic environment, generating appropriate treatment plans and providing therapeutic services to children and youth in collaboration with social workers, family, and team members. They are responsible to supervise and direct front-line support staff in their delivery of treatment case plans.
WORKING CONDITIONS: Full-time, working mostly business hours. May be required to work after hours for staff meetings, times of crisis, monthly educational/recreational evening programming or when scheduled as part of the on-call rotation. Work can be physically and mentally demanding. Self-care should be emphasized and encouraged.
QUALIFICATIONS:
- Post-secondary degree in Social Work is preferred - a post-secondary degree specializing in social sciences may be considered or a combination of education and relevant working experience may be considered;
- Minimum of four years working experience providing physical, psychological, and social support to at-risk youth and /or other vulnerable populations from a trauma-informed perspective is required;
- Minimum of one-year supervisory experience is required;
- Thorough knowledge of attachment theory, attachment styles and the impact of attachment disruption is essential;
- Demonstrated understanding on issues impacting youth presenting with mental health and other complex needs;
- Demonstrated experience and competence working independently and as part of a multi-disciplinary team, preferably working across agencies/systems in the provision of coordinated care;
- Knowledge and experience in issues and supports related to addictions, harm reduction strategies, and/or sexual exploitation preferred;
- Mental health/crisis training certifications including NVCI (Non-Violent Crisis Intervention) and ASIST (Applied Suicide Intervention
Skills Training) is considered an asset;
- Knowledge of planning, implementation and evaluation of client treatment plans;
- Must be comfortable and competent in crisis de-escalation and behaviour management;
- Comprehensive knowledge of children/youth services and resources available in the community;
- Valid First Aid/CPR is required;
- Valid class 5 driver’s license, access to a reliable vehicle, and satisfactory driver’s abstract;
- Satisfactory Criminal Record and clear Vulnerable Sector checks;
- Clear Child Abuse Registry and satisfactory Prior Contact checks;
- Knowledge of Indigenous culture, or willingness to learn, including participating in cultural practices, programming and ceremonies alongside youth;
- Must be available to participate on an on-call rotation; including supporting staff and youth in crisis; be willing to attend in-person support for housing concerns and other emergent situations.
- Excellent verbal and written communication skills;
- Demonstrated decision making and problem solving skills;
- Proficient knowledge of computers and experience with Microsoft Office software.
DUTIES AND RESPONSIBILITIES:
- Develops, implements, monitors and evaluates the operations of the program, ensuring compliance as per organizational requirements, Policies and procedures, and best practices for client-centered care within a safe and therapeutic milieu;
- Develops an awareness of each youth's individual psychological, educational, physical, spiritual, and social needs and goals, and actively promotes these on behalf of the youth; including organizing/facilitating weekly group programming.
- Provides clinical assessment, evaluation and monitoring of progress of the treatment care plan, present referrals to psychiatry, psychology, therapy and/or education to facilitate actualization of the goals set during the Treatment conference.
- Collaborates with Child and Family Services agencies, schools, clinical and medical services, the judicial system and other stakeholders, to advocate the best possible services for the youth in care, as well as their families;
- Discloses pertinent information and formal documentation related to child allegations or ongoing concerns to the appropriate child welfare authority and supporting CFS agencies;
- Guides and facilitates the implementation of strategies such as planned daily activities, coordinated treatment interventions, structured environments, and organized recreational and social activities;
