**JOB SUMMARY**

The Rehabilitation Support Worker (RSW) provides vital assistance to adults living with physical disabilities, post-stroke conditions, and seniors in achieving greater independence. The RSW works closely with clients to support their goals in daily living activities, personal care, and rehabilitation and delivers engaging programs that promote physical, cognitive, emotional, and social well-being for clients.

This role requires a compassionate, team-oriented professional who is committed to the principles of independent living and working collaboratively with a team of RSWs, Occupational Therapists and Physiotherapists to deliver holistic, client-centered care.

**KEY RESPONSIBILITIES**
- Design and deliver structured individual and group rehabilitation and activation programs based on client interests, strengths, and goals.
- Support clients in identifying, setting, and achieving meaningful participation goals, adapting programs to meet diverse needs and abilities.
- Encourage social interaction, community involvement, and full inclusion while promoting dignity, autonomy, and independence.
- Collaborate with the interdisciplinary team to align programming with individualized care plans, rehabilitation goals, and assessment outcomes.
- Educate clients on the benefits of activation, physical therapy, and recreation for recovery, health maintenance, and quality of life.
- Foster self-management and lifelong leisure skills to support continued engagement beyond the program.
- Assess the effectiveness of exercises and activities and adjust based on client progress and feedback.
- Create and implement individualized therapeutic recreation plans in collaboration with Occupational Therapists, Physiotherapists, and other staff.
- Participate in assessments, progress reviews, and planning meetings; monitor and document client engagement, achievements, and concerns.
- Assist clients in accessing community-based leisure and physical therapy opportunities.
- Plan and coordinate seasonal events, outings, and programs in partnership with community organizations.
- Maintain accurate documentation, including progress notes, client records, and case conference reports.
- Contribute to program development and evaluation through participation in team meetings and feedback discussions.
- Maintain program materials, supplies, and equipment to ensure a safe, organized, and accessible environment.
- Participate in ongoing training and professional development, including mandatory in-service sessions.

**QUALIFICATIONS**

**Education**
- Post-secondary education in Recreation Therapy, Occupational Therapy/Physiotherapy Assistant, Health Promotion, or a related field.
- Must have 2-5 years of experience in a related role. Alternatively, recent hands-on experience in rehabilitation or activation settings within the past five years will also be considered.

**Skills & Qualifications**
- Strong verbal and written communication skills; bilingualism in English and French is an asset.
- Demonstrated ability to model self-advocacy and decision-making aligned with the principles of independent living.
- Proven ability to work both independently and collaboratively within a team environment.
- Excellent interpersonal skills, with a focus on empathy, respect, and cultural sensitivity.
- Creative, flexible, and client-centered approach to program delivery.
- Valid driver’s license and access to an insured personal vehicle (required).
- Current First Aid certification (required); CPR certification (preferred).

**Experience & Assests**
- Experience working with adults living with physical disabilities or post-stroke conditions (preferred).
- Strong group facilitation skills.
- Prior experience working as part of an interdisciplinary team to deliver holistic care.
- Ability to communicate effectively with clients, families, and professionals across disciplines.
- Understanding of and commitment to independent living principles, supporting clients in achieving autonomy based on their individual abilities.
- Demonstrated ability to manage multiple responsibilities while maintaining thorough and timely documentation.
- Capacity to connect clients with relevant community resources and services.
- Critical thinking and adaptability in responding to evolving client needs.

**Working Conditions**
- Full-time, permanent position, generally Monday to Friday, with flexibility for training or other needs.
- May require driving in inclement weather.

**Physical Requirements**
- Heavy, repetitive physical tasks, including assisting with transfers, lifts, and mobility aids (e.g., wheelchairs, walkers).
