**The Mint Health + Drugs**: CMP's Mental Health and Addictions
- Outreach Nursing Team is a nurse-led outreach team dedicated to engaging individuals experiencing health and social inequity. The home care team manages a caseload of approximately 30 clients, operating 7 days a week, 365 days a year to provide home care services with a tailored focus towards medication administration and chronic disease monitoring and follow-up. The majority of clients reside within housing programs, shelters, and facilities across Edmonton that are staffed by the non-profit organizations we work with, however, a small subset of individuals also reside within independent living settings (e.g. apartment complexes, houses, etc.).
Key therapeutic areas include Mental Health, Substance Use Disorders, HIV, Hepatitis C, and Chronic Disease Management.
**Scope of Services**:
- Comfort in independently assessing and dosing witnessed medications such as methadone, Suboxone®, and Kadian®
- Communicate clearly with patients, peers, management, and other healthcare professionals through a variety of media using effective and appropriate communication skills.
- Coordinate appointments, access to care and social services for clients.
- Advise the patient to seek further medical attention where appropriate.
- Answer any questions requiring clinical knowledge, which fall within the scope of practice as a licensed practical nurse in Alberta.
- Maintain confidentiality of patient information.
- Provide patient health education, harm reduction, and employ principles of preventative medicine.
- Provided clinical care as it relates to physical assessment, minor ailments, medication therapy, and activities of daily living where required and within scope of practice.
- Follow principles of person-centered care when providing wholistic care.
- Provide wound care and maintain the cleanliness and stock of supplies and equipment.
- Ensure accurate and timely completion of appropriate documentation, including client charting in an eMAR (electronic medication administration record).
- Provide leadership in the management of client care.
- Coordinate and communicate with a multidisciplinary team to demonstrate patient centered care.
- Demonstrate willingness to learn new skills as required

**Mint Health + Drugs**: CMP(Community Members Pharmacy) was opened in 2012 with the mission of serving our community members within Edmonton’s vulnerable population sector.
Over the years, our pharmacy has grown extensively, and today, we have the privilege of working alongside the most prominent non-profit organizations in supporting this population that truly needs it the most.
Though we’ve since relocated our pharmacy into the Britannia-Youngstown neighbourhood from our original location in an effort to extend our support to a new community, our core principles remain unchanged. That is, we value the importance of listening, and understand that every person is unique having come from all walks of life carrying their own personal story. That’s why we strive hard to take the time to listen to every individual we have the opportunity to come across so that we cater our support around wherever they are in life and wherever they need us the most.
Whether it’s helping them find the best treatment option for their concerns, managing their medications, connecting them with a resource for sustainable housing, or just simply being a listening ear to hear out their frustrations, we’re always here to help.

The LPN is the lead clinical service provider for the chronically homeless population who are members of a private housing group in Edmonton. The LPN is expected to meet and exceed the standards of patient centered care as services are performed within the scope of practice for an LPN in Alberta. There is a focus on ensuring service and clinical excellence.
